Objective To define the procedure of bone marrow aspiration used to diagnose visceral leishmania Scope The Sop describes the steps for bone marrow aspiration from material preparation to specimen collection and patient observation post procedure Process - Materials required o Bone marrow aspiration needle with 10 cc syringe o Clean microscope slides o NNN culture medium o Lidocaine o Syringe with needle 5cc o Spirit lamp with sufficient flame o Sterile drapes o Sterile gloves o Sterile cotton and gauze o Iodine solution for cleaning o Plaster, labels, pen/pencil - Indication for diagnosis of visceral leishmania, preferred procedure when splenic aspiration is contraindicated o It can be done from sternum or iliac bone - Aspiration procedure o Explain the procedure and indication to the patient and get consent o Let patient lie supine to take from sternum or lateral position to take from iliac bone o Palpate the appropriate site for puncture, the upper part of the body of the sternum or prominently palpable iliac crest o Put on sterile gloves, clean the site of puncture with iodine and put a clean drape o Infiltrate the area to be punctured with lidocaine layer by layer from the skin to the periosteum o Puncture into the bone vertically with care to put the tip of the aspiration needle into the bone marrow and aspirate using a 10cc syringe slowly pulling the plunger o Remove the syringe from the needle as soon as an aspirate is just visible in the syringe o Heat the edge of the culture media containing tube and put a tiny drop of the aspirate in to the culture tube Page 1 of 2
o Put a drop of aspirate on to the clean slides and make smears o Smear should be done without a delay after aspiration to avoid clotting of the aspirate o Remove the bone marrow aspiration needle, clean the site and dress o Label the specimens, both the culture tube and the smears. Then hand over for laboratory process o Observe the patient for a while until comfortable, no pain Responsibility o Bone marrow procedure is a team work o Nurse prepare materials required for bone marrow aspiration, the patients medical documents, follow and report vital signs after the procedure o Laboratory technician label the slide and culture media, smear, stain and examine slides under microscope. S/he will also inoculate culture and report the result. o Physician the aspiration procedure will be done by the physician. S/he will also document the procedure and do follow up.
