PRUBSN's Hospital Alliance Services Guideline

PruBSN's Hospital Alliance Services allows customers to use participating hospital networks after the first 3 months of their major medical coverage. During the first 3 months, customers must pay hospital bills in full and submit documents for possible reimbursement. Exceptions are made for major accidents where the network can be used immediately. PRUBSNs Hospital Alliance Services

Welcome to PruBSNs Hospital Alliance Service. The Hospital Alliance Services will be available to you, subject to the Companys terms and conditions, from the fourth (4) month of your Major Medical Cover date. Should you require admission for medical or surgical treatment during the first three (3) months of cover, you are required to settle the hospital bill in full. You may submit the required documents to PruBSN for consideration for re-imbursement. Exceptions however are given to admission necessitated by major accidents where you may proceed to use the Hospital Alliance Services during the first three (3) months of benefit cover. ADMISSION PROCESS (during operating hours 8.30 am to 9.00 pm) 1. In event of hospitalization, the medical officer or Specialist is required to complete the Medical Report Part I form. 2. Please present your Medical Card upon admission for verification process at the hospital admission and discharge counter. 3. Certificate owner is required to read and sign the Declaration & Authorization Form. 4. Hospital will then fax the Medical Report Part I together with the Declaration & Authorization Form to PruBSN. 5. PruBSN will issue the initial guarantee letter to hospital subject to terms and conditions of takaful certificate.
Note: Hospital may collect Excess Deposit to cover non-medical expenses and co-takaful charges during admission. Hospital may also collect Admission Deposit if pending initial guarantee letter from PruBSN.

DISCHARGE PROCESS (during operating hours 8.30 am to 9.00 pm) 1. Upon customers discharge from hospital, the Specialist is required to complete the Medical Report Part II form, and hospitals billing department to prepare and furnish the final hospital bill. 2. Hospital will then fax the Medical Report Part II and the final bill to PruBSN. 3. Upon approval, PruBSN will issue the final guarantee letter to hospital subject to terms and conditions of takaful certificate. 4. Hospital will received the final guarantee letter and certificate owner is required to very and sign the final bill.
Note: Hospital may refund the deposit or any balance at their discretion. Hospital may collect the non-medical expenses and co-takaful charges upon discharged based on certificate terms and conditions.
