Medications Conversions (IV to PO) Recommendations

Oral therapy should be considered in any patient receiving intravenous medications and capable of
taking oral medications (i.e. eating, taking other PO medications, adequate gut function). Below are
recommendations for conversion of IV medications to PO medications. Please note that oral therapy
may not be appropriate for all patients (i.e. oral regimens do not achieve adequate serum
concentrations). Lastly, a considerable cost savings may be realized by promptly changing patients to
oral therapy when indicated.
