Change Order - Blank Template

This change order document modifies an existing subcontract agreement between a contractor and subcontractor for a construction project. It lists the change order number, date, original contract date and project name. The description of changes section notes the scope of modifications to the original agreement. Financial amounts are provided for the original contract sum, net changes from previous change orders, contract sum prior to this change order and the impact of the current change order on the new total contract sum.
