Chicken Pox NCP

The patient presented with acute pain related to an inflammatory process, exhibiting weakness, limited range of motion, restlessness, facial grimaces and irritability. Through nursing interventions like establishing rapport, monitoring vital signs, encouraging pain reduction techniques and administering analgesics, the patient reported a decrease in pain within 3 hours. The second patient presented with a risk of infection from scratching papulovesicular lesions, with pruritus, high white blood cell and leukocyte counts. Nursing interventions like monitoring status, encouraging wound cleaning and a nutritious diet, and administering antiviral medication aimed to keep the patient free of infection. After 4 hours the patient showed no signs of infection.

Diagnosis Acute Pain related to inflammatory Process

Planning Within 2-4 hrs of nursing interventions the pt. will report decrease pain.

Interventions y Establish rapport

Rationale >To gain the trust and cooperation of the patient >To provide baseline data and note deviations from normal. >Helpful in establishing diagnosis and treatment needs >To reduce pain and promote relief/comfort >To promote healing >For clients comfort and relief from pain >To decrease pain.

Evaluation After 3 hours of nursing intervention, the patients level of pain decrease.

Monitor and record vital signs.

Review factor that aggravate or alleviate pain

Encourage pain reduction techniques

Provide adequate rest Provide diversional activities like socialization Administer analgesics to maintain acceptable level of pain if not contraindicated

Instruct client to perform deep breathing exercises (DBE)

>Deep breathing exercises may reduce pain sensation/ used in pain management >To promote timely intervention/ revision of plan of care

Monitor effectiveness of pain medications

Planning After 2-4 hours of nursing intervention, the patient will be free from infection.

Interventions Monitor patients status.

Advised the patient not to scratch the wound Encouraged regular cleaning of the wounds. Advised the patient to eat healthy or nutritious foods especially those rich in vitamin C.

Rationale >To determine the baseline data for future comparison. >To avoid infection.

Evaluation After 4 hours of nursing interventions, the patient is free from any signs of infection as evidenced by: >no fever >no inflammation

>Good hygiene promotes faster healing. >This helps in patient recover and boost the immune system.

Advised client to stress proper hand washing. Encouraged personal hygiene like regular bathing. Advised client to apply Calamine lotion on the affected area to avoid or decrease itchiness. Administered anti-infective drug like Acyclovir 8mg as prescribed by the physician. Cut fingernails short.

>To avoid spread of microorganisms. >To decrease or relive itchiness.

>Calamine lotion helps in lessening itchiness cause by bacteria.

>To prevent infection and further complications.

>Bacteria resides under the nails.
