KNOW ALL MEN BY THESE PRESENTS:


THAT I, _________, of legal age, Filipino citizen, with
residence and postal address at Aurora Village,Tumaga Zamboanga City,
Philippines, do hereby APPOINT, NAME and CONSTITUTE _________, of
legal age, Filipino citizen, and with residence and postal address at
Rm. 7 Laiko Bldg., 372 Cabildo St. Intramuros, Manila, Philippines,
to be my true and lawful attorney-in-fact, for me and in my name,
place, and stead, ACTING JOINTLY OR INDIVIDUALLY, to DO and EXECUTE
all or any of the following acts, deeds and things, to wit:
1. To represent me before the Bureau of Customs, Manila,
Philippines, in my capacity as a retiree, to claim and receive
my terminal leave check(s) due from the said office;
2. To sign for and on my behalf all documents and papers
pertaining to said withdrawal of check(s) from the aforestated
office;
3. To deposit said check(s) to my account with the Landbank
of the Philippines;
4. To do all acts necessary and reasonable for the said
purpose.
GIVING AND GRANTING unto my said attorney-in-fact full power and
authority to do and perform every act and thing whatsoever requisite
and necessary to be done as fully to all intents and purposes as I
might or could do if personally present, HEREBY RATIFYING and
CONFIRMING all that my said attorney-in-fact shall have done,
lawfully do, or cause to be done by virtue of these presents.
