BIRTH OF CHANDRASEKHAR KADIYALA


I, the undersigned, Mr.Ramakrishnaiah Musunuru, aged - 62 years, Hindu by religion,
residing at Plot #673, Hmt Hills, Kukatpally, Hyderabad, Telangana, 500085, India, do
hereby solemnly affirm and declare on oath that:
1. I am the deponent herein and a citizen of India.
2. I was born on April 23rd, 1953 in Ammavarigudem, Krishna district, Andhra Pradesh,
India.
3. My sister deceased ShanthaKumari Kadiyala (mother of ChandraSekhar Kadiyala ) was
married to Mr. NageswaraRao Kadiyala on March 12th, 1977 in Hyderabad, Telangana,
India.
4. ChandraSekhar Kadiyala was born on Nov 1st, 1981 in Hyderabad, Telangana, India. I
have the personal knowledge of ChandraSekhar Kadiyala's birth because I am his uncle
(mother's brother) and we are very close relatives and recall ChandraSekhar Kadiyala's
birth clearly as I was present at the time of his birth.
5. It is a religious custom in India to name the child after its birth in a religious ceremony.
Therefore, ChandraSekhar Kadiyala's, official record of birth was not filed immediately.
So I do hereby solemnly affirm that foregoing are true and correct statements.
