You are being given the opportunity to evaluate the mistakes you made on a test. Rework the problems
that you missed to improve the grade that you earned for that test.

The following must be done to qualify for a maximum re-test grade of 75.
1. Rework all incorrect problems. You may use your book, class notes, and attend tutorials.
2. Write the specific reason you missed the problem. (misread directions, used the wrong formula,
didnt multiply a negative times a negative, did not distribute the -5, etc.)
3. Rework the problem showing all steps.
4. Write the new answer clearly in the answer column.
5. Additional corrections may be done on notebook paper using the same format shown below or by
printing additional copies.
6. Have your parent sign this document and the test.
7. Staple this document to your test and return it by the due date.
