I, LEO D. LLACUNA, of legal age, married, Filipino and residing at 16 San Jose St., Polillo,
Quezon, under oath, hereby depose and say:

1. That I am the General Manager of LD LLACUNA CONSTRUCTION & SUPPLY, with office
at 16 San Jose St., Polillo, Quezon;

2. That I have inspected the site for Construction of PBLARC MPC PHF (Post Harvest
Facility), located at Brgy. Libjo, Polillo, Quezon on July 30, 2013;

3. That I am making this statement as part of the requirements for the Technical Proposal
of LD LLACUNA CONSTRUCTION & SUPPLY for the bidding of the Construction of
PBLARC MPC PHF (Post Harvest Facility) at Brgy. Libjo, Polillo, Quezon.
