Choose Form

This document is a form for graduate students admitted to multiple degree programs to choose which one program they intend to complete. It allows them to indicate their choice to the Graduate College and continue registering for classes in that program. The form has sections for student information, enrollment option selection, and submission. Students can choose to enroll in only one program, complete all programs concurrently with department approval, or withdraw from their current program and switch to a new one.

CHOOSE FORM
Fill out this form when you have been admitted to two or more graduate degree programs. It will
allow you to indicate your intentions to the Graduate College and allow you to continue to register in
your chosen program.

Part II: Choose ONE of the following enrollment options

option, you must also attach memos from each department giving their approval of your
concurrent attendance.

option, you must also attach the Voluntary Withdraw form indicating your withdrawal from
