1) This document is an application form for an individual to reactivate their username for State Bank of India's internet banking service called "OnlineSBI". 2) The applicant provides their name, account number, email address, residential address, phone numbers and signature to request reactivation of their username which had been deactivated. 3) Bank staff will verify the applicant's details, note the request in the user creation register, and approve or reject the reactivation of the username.
