Delayed gastric emptying and pulmonary aspiration following induction of anesthesia in patients using GLP-1 drugs was evaluated across over 23,000 patients by Dixit et al. in JAMA. Just about three and half thousand were on GLP-1 medications, and there was no significant difference in the incidence of postoperative respiratory complications between those on a GLP-1 drug versus those not on the drug. The authors suggest ‘…liberalizing the withholding guidelines for GLP-1 RAs preoperatively should be considered.’ Whilst a small study, I do think that anyone on a GLP-1 medication should inform their clinician, especially if they are to undergo a procedure under sedation or general anesthesia. https://lnkd.in/eF6w5M-F

Finally enabling effect-site target-controlled infusion (TCI) for more precise ketamine delivery during anesthesia? 🔑 Key Findings: 📊 After a single bolus of ketamine (0.1 mg·kg⁻¹), ANI values increased from 38.5 ± 4.95 to 53.5 ± 4.95, with a peak effect at 1.83 ± 0.74 min. ⏱️ The time lag between plasma concentration and ANI response (hysteresis) was modeled with an estimated Ke0 = 0.238 min⁻¹. ✅ The PKPD model, incorporating Domino PK parameters, successfully described ketamine’s temporal profile. 💡This research highlights the potential for integrating this model into an effect-site TCI system, paving the way for more precise and individualized analgesia management.

Thus, placebo-controlled antiobesity medication studies include monthly, individualized lifestyle counseling as their control to reflect this standard in care. There may be a physiologic mechanism that supports this approach, as GLP-1RA may reinforce behavioral strategies by suppressing food reward pathways and enhancing satiety signaling. Based on this evidence base, guidelines continue to recommend antiobesity medications to be used only in conjunction with lifestyle intervention in a stepped-care approach”

In this secondary analysis of the Safety, Tolerability, and Efficacy of Rapid Optimization of Heart Failure Therapies (STRONG-HF) randomized clinical trial including 515 patients, 2 weeks after discharge, medium- to high-dose GDMT was prescribed in more than 90% of patients. Patients with lower blood pressure, more congestion, and characteristics denoting higher risk were less uptitrated; patients prescribed more GDMT had lower rates of readmission for HF or death through 6 months and more improved quality of life. What this means is that when patients with AHF can tolerate higher doses of GDMT, results suggest that all efforts should be made to rapidly uptitrate to optimal doses of the 3 and (likely) 4 pillars of HF medications, including renin-angiotensin receptor system inhibitors, β-blockers, mineralocorticoid receptor antagonists, and sodium-glucose transport protein 2 inhibitors
