Hawaii this summer is poised to roll out palliative care coverage via Medicaid. In May, a State Plan Amendment (SPA) that allows coverage of palliative care through Med-QUEST, the state’s Medicaid program, was approved by the U.S. Centers for Medicare & Medicaid Services (CMS). The Medicaid unwinding process has highlighted significant challenges, particularly in exacerbating health disparities and inequities. Many beneficiaries lost coverage due to procedural issues, such as changes of address or communication barriers, which disproportionately affected Black and Hispanics as well as individuals with limited English proficiency. This has led to increased uninsured rates among vulnerable populations. State Medicaid agencies across the nation were overwhelmed, struggling with staff shortages and outdated eligibility systems, further complicating the renewal process. These issues aren't just theoretical; they've had real-world impacts, including right here in Oklahoma, where many residents have been left without coverage. As we move forward, it's crucial to push for policy changes that address these disparities and ensure equitable access to healthcare for all.
