"Intentional Replantation: 5-Year Follow-Up Success" Intentional replantation is a surgical procedure designed for cases where conventional endodontic treatments fail or are not feasible. The technique involves the deliberate extraction of a tooth, extraoral inspection, repair of root surfaces, and replantation into its original socket. Its key indications include: * Failed nonsurgical root canal treatments. * Complex root resorptions or perforations. * Teeth with crown-root fractures or inaccessible anatomical defects. * Financial constraints preventing extensive treatments like implants. The procedure benefits from direct visual access to root surfaces for repairs while preserving natural dentition. However, success hinges on careful handling to protect the periodontal ligament (PDL) and minimize extraoral manipulation time, ideally under 15 minutes, to prevent resorption and ankylosis. Recent advancements, such as using materials like bioceramics and techniques like apical root resections and retrograde fillings, have improved outcomes. Systematic reviews report survival rates nearing 90%, making it a reliable and cost-effective alternative in select cases. This case, with a 5-year follow-up, showcases excellent periodontal healing, no signs of root resorption, and stable functionality—reinforcing the predictability of this approach when conventional options are unfeasible.
