Sometimes we run into situations where brokers or customers are calculating our PMPM fee relative to the number of visits we anticipate completing. I totally understand why. It's their most common frame of reference. But there's an underlying problem with this thinking: The traditional system for PT rewards inefficiency. Our model is fundamentally different. We stratify risk upfront so patients get the RIGHT amount of care. Low-risk patients can go back to living life. We aren't motivated to keep them around because they are "easy" and "billable." They may only need one visit! High risk patients / more complex cases can stick around as long as they need. Not just that, but it's in our best interest to create sustainable change by treating the root cause of the pain so they don't actually need to come back. Less visits = better. Let me caveat this statement. MSK remains a HUGE category, so there will always be demand and visits. If we are getting 0 visits, we aren't doing a good job on engagement. 50% of the population has an MSK issue at any given time, so we better be delivering at least 15% engagement...in our case, our average is 25%. But once that person is in, you have to understand their risk profile and use that to shape the treatment plan which doesn't always need to be 10-12 visits.
