A new service which helps patients – or a friend or family member – escalate concerns if their condition begins to deteriorate has been launched today, Wednesday 24 April. Call 4 Concern is a patient safety initiative which is being led by our Critical Care Outreach Team – a team of experienced specialist nurses who support acutely ill patients across our hospital. The scheme will enable adult inpatients and their significant others – including a partner, next of kin, family member or a friend – to have a clear route to accessing advice if they have concerns regarding a noticeable change or deterioration in their or their loved ones condition. Call 4 Concern – C4C for short – encourages you to speak to the ward nurse or doctor in the first instance but with the option to then make a call for further assistance if this is required. If there is a concern that a patient is becoming more unwell a member of the Critical Care Outreach Team will visit the ward to assess and then liaise with your nursing and medical team. C4C is purely for concerns about a noticeable change in the medical condition of an adult patient – not for more general issues. “Impact” is ProxsysRx’s readmission reduction program, using a clinical pharmacist in collaboration with a nursing team. It starts at discharge, when the pharmacist conducts a Comprehensive Medication Review bedside — and it continues with a Patient Coordinator / Nurse reaching-out to a patient at least four times over the course of 30 days, post-discharge In the program’s first month, for patients enrolled in the program, readmissions were 55% lower than the health system’s historical rates. In 10 months, the health system saved $486,400 (a figure based on DHS’s estimated average readmission costs), and patient satisfaction among those enrolled was rated 9.77 out of 10. A Becker's article about one systems test to try and share the responsivity of patient care and involve RN's and NP/PA's more in the interaction in the hospital. Makes sense to me.... if this model grows, the need for Advanced Practice professionals will grow... and as you know, NP's and PA's fall under Locums. This is why Locums will grow at the faster rate than nursing. Under this system, a registered nurse manages the initial triage, while an APP serves as an "inboxologist." "We piloted an inbox model where in-basket messages — phone calls, patient advice requests, prescription requests, second-level triage requests, all of the MyChart messages that were coming in — would actually be triaged in two ways," Dr. Inman said.  "First of all, by an RN with clinical competency that would kind of scrub things, and anything that needed to go to a provider would then go to an APP pool.
