Congratulations to NACHC’s Million Hearts® team (Meg Meador, MPH, C-PHI, CPHQ), and the Waianae Coast Comprehensive Health Center in Hawaii for winning the Techquity Insights award and the Techquity Action Award at last month's ViVE conference! NACHC's Million Hearts® initiative, awarded for both significant insights and impact, targeted uncontrolled hypertension in Black and African American adults, populations which have experienced lower blood pressure control rates and higher prevalence of hypertension compared to White adults. Led by NACHC in collaboration with primary care associations, health center-controlled networks, and community health centers, this initiative seeks to: 1. increase adoption of evidence-based hypertension diagnosis and management practices 2. promote the use of combination therapy over monotherapy for hypertension management, a method proven to be more effective in controlling blood pressure and reducing cardiovascular risks with fewer side effects. 3. utilize treatment intensification process measures to identify gaps and drive optimal prescribing practices to improve blood pressure control outcomes and contribute to the broader goal of advancing health equity Through this initiative, NACHC has already improved blood pressure control by 13.2% from July 2021- June 2023 among Black and African American adults with hypertension in participating health centers and aims to achieve a 20% improvement in blood pressure control by July 2024. Public health appreciation post (though I may be biased, ha)! It’s easier to quantify the number of diseases diagnosed and treated than it is to measure the number of illnesses prevented. We can easily see the impact of a new cancer drug that shrinks tumors or a surgical procedure that saves lives. But how do you measure the impact of clean water initiatives that prevent cholera outbreaks, vaccination campaigns that eradicate polio, or public education programs that reduce smoking rates? These are the unsung victories of public health, preventing illness and suffering before they even have a chance to take hold. While sophisticated modeling techniques exist to estimate the impact of public health interventions, quantifying the absence of disease is challenging. These models, while valuable, often rely on assumptions and indirect measures, making them less precise than the concrete data obtained from diagnosing and treating existing illnesses. This inherent difficulty in measuring prevention’s success further contributes to the undervaluation of public health efforts. Throughout history, public health measures have been instrumental in improving human health and longevity. From sanitation and hygiene practices to vaccination and disease surveillance, public health interventions have saved countless lives and prevented untold suffering. Yet, because these successes often involve preventing negative outcomes, they are less visible and less celebrated than the dramatic cures and treatments of clinical medicine. This lack of recognition has real-world consequences. Public health departments are often underfunded and understaffed, struggling to carry out their essential work. When funding cuts are made, public health is often the first to suffer, as its impact is seen as less immediate and less critical than that of hospitals and clinics. Public health is not just about preventing disease outbreaks or responding to emergencies. It’s about creating healthier communities, ensuring access to clean water and sanitation, promoting healthy behaviors, and advocating for policies that protect the health of all people. #PublicHealth

Yet, despite these advancements, our health challenges have grown exponentially. Chronic diseases are skyrocketing, with the World Health Organization forecasting that by 2050, these conditions will account for 86% of the 90 million annual deaths worldwide. What if hospitals where paid to keep patients healthy? This is now possible. Thought provoking article and a call to action to all the healthcare system agents. Citing the authors Nicole Sirotin, MD DipABLM FACP & Needa Qureshi “Once the healthcare system encompasses lifestyle interventions on a prescription basis, chronic illnesses can be prevented, treated better and often reversed, which will reduce the global burden of disease and healthcare spending” What are you willing to do to make this change a reality? All contributions are welcome, no matter the size. We are in the verge of a paradigm change as big as the discovery of antibiotics or the use of antisepsis.

Great job completing the NTM/MAC survey! 👏👏👏 I recently learned just how crucial these surveys are. The FDA requires patient input before researchers can move forward, ensuring "patients' experiences, perspectives, needs, and priorities are meaningfully incorporated into decisions about their health." This empowers patients! I'll share from experts as I learn more, but this is how we make a difference. Some of us were ineligible for the survey which can be frustrating. We want to contribute, and being told "no" feels like rejection. Often, researchers need very specific patient groups, so please don’t take it personally. Perhaps at some point, surveys will not only message us saying we’re ineligible, but might even explain why. This would certainly make us feel better and encourage us to participate in the future. The more we participate, the faster research advances. Imagine if research were constantly stalled due to lack of participation by patients - treatments for rare diseases could become too expensive to pursue. We must show that we are ready and willing to contribute and for many, surveys and interviews are comfortable ways to do so. While I love discussing exercise, airway clearance, and healthy eating, my focus will increasingly be on research and advocacy. Remember, progress requires participation. The newly launched 10th edition of the Red Book, in partnership with St Vincent’s Health Australia and the RACGP, brings crucial updates to support patients dealing with overweight and obesity. This edition encourages GPs to adopt a holistic approach, addressing not only the medical aspects of obesity but also the barriers to healthcare access, stigma, and discrimination that many patients face. The updated metabolic medicine chapter highlights strategies for preventing chronic diseases through better management of nutrition, obesity, and physical activity. Given that “In 2022, two in three (66%) of Australian adults aged 18 and over and one in four (26%) of young people aged 2–17 were living with overweight or obesity,” and Australia ranks 10th out of 21 OECD countries for this issue, it’s vital that we stay informed and proactive.
