“Insanity is doing the same thing over and over again and expecting different results.” A quote often attributed to Albert Einstein but more accurately it was penned by civil rights campaigner and feminist writer Rita Mae Brown. The NHS faces unprecedented challenge and more than ever needs to acknowledge the need for transformation. Simply doing what it has always done better will always be insufficient. It is time to fulfill the ambition of “subsidiarity” and invest in the workforce to lead change from the bottom-up. This should then determine what excellence of leadership and management looks like in more centralised functions. Katrina Percy makes the case for just this in integrated neighbourhood working in the attached article. We cannot simply repeat the top down followed by the bottom-up approach when resources allow within the NHS.
