Interesting intrigue around the 4-star to 3.5 star 5% PMPM (HUGE for #healthplans) quality bonus #Obamacare cliff. Proposed #CMS changes may help the perennial 3.5 star plans (the #Centene's and #Molina's of the world) and hurt the 4.0 and 4.5 star plans (the nonprofits #Kaiser's, #UPMC's, #Providence's, etc). $20B in bonuses is in play over 10 years. Looks to be best to attain and maintain 5-Stars whenever possible (consistently demonstrate the absolute best care for the population served, demonstrate the best taxpayer value and the Obamacare-granted power to market for new membership the year around). #Darwinianism rules! #SDOH #HealthEquity #HealthcareReform #HealthcareTransformation #PrimaryCare #PopulationHealth #PatientCenteredCare #PatientCenteredHealthcare #HealthcareQuality #ValueBasedCare #ValuebasedHealthcare

https://lnkd.in/ga2pct_z #EndOfLifeCare #PalliativeCare #HospiceCare #VoluntaryAssistedDying How do we as communities of care and support within our nation ensure the best possible and most appropriate service provision for our most vulnerable older people. And what might that look like? Now a major topic of concern in the UK too.
Government expected to help UK hospices hit by national insurance rise

SeaView believes in providing accessible behavioral healthcare for ALL. That's why we offer a Financial Assistance Fund! ⬇️ Who is this for? • Individuals seeking behavioral health services who do not qualify for SeaView’s Sliding Fee Scale and who are not eligible for Alaska Medicaid. • Most individuals seeking financial assistance in this category would be identified as middle income or hold commercial insurance plans, but are considered underinsured by having high deductibles and co-pays for services. What does it do? • Provides up to $750 in assistance to qualified individuals. • Ensures that qualified individuals can begin the process of accessing needed behavioral health services with limited out of pocket costs. How to apply! • Inquire at your intake appointment. • Complete the application with our admissions team prior to your first clinical appointment. • Follow up! Give us a call at (907)-224-5257 to inquire about your application status. *This fund was made possible by a grant from the Seward Community Foundation* #AssistanceFund #BehavioralHealth #Healthcare #Accessibility #SeaView #SewardAK #AlaskaCommunity #Apply #Inquire #Eligibility #Grant

When we talk about health, it's not just about doctor visits or medications. Health is deeply influenced by the conditions in which we live, work, and play—these are known as social determinants of health (SDOH). Social determinants of health include factors like: ▪️ Access to healthcare services ▪️ Education and job opportunities ▪️ Safe housing and neighborhoods ▪️ Income level and economic stability ▪️ Access to nutritious food ▪️ Social support and community networks These factors can have a profound impact on a person’s well-being. For example, someone living in a food desert may struggle to find healthy food options, leading to higher risks of chronic diseases. Similarly, a lack of affordable housing can create stress and instability, which negatively affects mental and physical health. Healthlincs and its partners offer care management for special needs children and adults with chronic illnesses, often facing gaps in insurance coverage and support for social determinants of health. To address these needs, the Healthlincs Care Resource Fund, established in 2020, provides financial assistance for services and products not covered by insurance, aiming to reduce health disparities beyond promoting healthy choices. This NPR article about #CalAIM and its impact on nonprofits hits home. Who is really benefitting from CalAIM and #Medicaid innovation? So far, it's the cottage industry of companies that are implementing changes. The benefits haven't really trickled down to frontline nonprofit workers, and haven't made a dent yet on root causes of poverty and race and class inequity. #socialcare #sdoh #healthequity #socialimpact
We at One Degree have observed firsthand some of the issues raised in this article regarding the CalAIM roll-out. The core problem: Top-down implementations of well-meaning health and social care policies often perpetuate and amplify power imbalances. Large healthcare organizations and for-profit companies drive policy, while community-based organizations and frontline providers—underpaid and understaffed—are left to do the work. This should also serve as a call to action for all of us in the social services sector. We need to advocate for more inclusion and equitable participation in policy discussions to ensure that the voices of frontline staff and the communities they serve are not only heard but become central to shaping solutions. It’s time for policymakers, funders, community-based providers and community voices to be at the decision-making table together. Will you join us in bringing attention to this issue and pushing for change? Please share your thoughts in the comments, send a message, or share this post to show your support! Whether you are a parent, school official, health practitioner, faith community leader, employer or government leader, you are in a unique position to affect change. This guide is designed to help you create healthy environments and to help you actively lead, advocate for, and implement healthy weight programs in your homes, your workplaces and your communities throughout the state of Virginia. Obesity is a disease that leads to other diseases. It shortens lives and increases health costs, including health insurance premiums. Overall, 28% of Virginia adults have obesity. Rates for African Americans and Latinos are higher than the state average at 41% and 29% percent, respectively. In comparison to other states, we rank in the top percentile, which means many other states have worse rates of obesity; however, we need to be proactive to address our needs here in the Commonwealth. Obesity rates in Virginia vary by community, ranging from 18% in some rural communities to 45% in our wealthier urban areas. Virginians need to continuously monitor the multiple factors (individual behaviors as well as environments where people live, learn, work, worship and play) that contribute to overweight and obesity among children, youth and adults. We have worked on a Revised Action Plan for a Healthy Virginia and it will be released soon! Want to get it before everyone else? This past year, #California spent $152 billion on Medi-Cal. One-quarter of that amount came from the state’s General Fund. In January, California became the very first state to open its Medicaid program, called Medi-Cal, to every undocumented immigrant within its borders. Some 700,000 adults between the ages of 26 and 49 now qualify for publicly funded health coverage. More than 300,000 undocumented individuals crossed the border between Mexico and California last year alone. If that trend continues, taxpayers, who already subsidize Medi-Cal coverage for 14.6 million Californians, could eventually be on the hook for more. https://lnkd.in/gmjspQUE
If expanding quality health care access is California's goal, Medi-Cal is not the solution

The Cigna Group Foundation's mission is to improve health equity and mental health but get this: They don’t allow their grant to fund the fringe benefits of the people enacting the work.… LOL. Let’s lose the hypocrisy of wanting to solve a health equity crisis while not supporting the health benefits of those solving the health crisis, ok? It’s enough of a #CrappyFundingPractice that you don’t fund administrative expenses, Cigna, but to draw the line at funding salaries while excluding integrated costs like benefits and payroll taxes? Have you never seen what goes into a nonprofit budget?! ARGH. How is it necessary that we need to say this: When you fund a nonprofit, you’re funding the humans that work there, and those humans deserve health care, and need unemployment insurance, and whatever FICA is…. They cost more than just their salaries. Get it together, Cigna! Are #CrappyFundingPractices depressing you, too? Send them our way: https://lnkd.in/eHcGHWrp
