Very interesting move by J&J with TREMFYA in PsO.  With these Phase IIIb SPECTREM study data, J&J was able to show Tremfya (guselkumab) achieved a 74.2% clearance rate in patients with low BSA psoriasis affecting special sites like the scalp, face, skin folds, and genitals, compared to 12.4% for placebo. These patients are often less likely to receive systemic biologics (versus topicals) since their psoriasis is covering less surface area. Nevertheless, the psoriasis can be uncomfortable based on the location affected. This move could potentially increase the size of the biologic treatment "pie". It will be interesting to see how the FDA and insurers react to this option for patients. It will also be interesting to see how competitors respond if this tactic proves successful. After years of inadequate treatments for moderate-to-severe HS patients, Novartis' Cosentyx is a welcome addition to the HS market. Based on our second quarterly tracker, dermatologists continue to perceive Cosentyx positively, especially when compared to AbbVie's Humira on nearly all inquired efficacy metrics. Further, these positive perceptions have increased compared to the last wave. Excitement for the newest biologic will likely continue until UCB's Bimzelx is approved and Cosentyx is faced with strong competition. Curious what solutions my network recommends to aggregate calendars from multiple accounts to have a consolidated view but keeping it all private and also having a feature where you can share a scheduling link that only shows pre-set open times for scheduling on demand. Never stop learning. Found this very interesting course on Linkedin learning, called "Advanced Content Marketing” by Madecraft and Kelli Schutrop! **LAG-3 Stumbles (again)** Yesterday, Merck announced that the phase III KEYFORM-007 trial had not met the primary endpoint of overall survival (OS). This trial was for its LAG-3 asset favezelimab (in a fixed dose combination with Keytruda), and had meant to support the lead indication: previously treated PD-L1+ microsatellite stable (MSS) metastatic colorectal cancer. With this newest "next-generation IO" setback, it's worth revisiting LAG-3's history and where the clinical landscape currently sits. The initial approval of Opdualag (a fixed dose combination of the anti-PD1 nivolumab and LAG-3 relatlimab) in 2022, LAG-3 “won” the race in becoming the next immune checkpoint inhibitor target. However, in the ensuing years it has not yet revolutionized the treatment of solid tumors outside of metastatic melanoma.    Opdualag is gradually becoming a standard of care in the US for metastatic melanoma (achieving H1’24 sales of $421M, +58% YoY), however the narrower label granted by the EMA has blunted usage ($20M in H1’24 vs. $4M YoY).  Its phase 3 trial failure in MSS PD-L1+ mCRC announced in Dec’23 also poured cold water on the broader role it might play in solid tumors (incidentally, this was the same patient population as in KEYFORM-007 so favezelimab's failure is not entirely unexpected).  Nevertheless, phase 2 data presented at ESMO’24 for PD-L1+ NSCLC are keeping the hope alive as BMS announced the initiation of a phase 3 trial for this indication.  Opdualag also has phase 3 trials underway for adjuvant melanoma and a subcutaneous formulation, as well as a phase 2 trial in 1L advanced/metastatic liver cancer. Like TIGIT, the early LAG-3 data for relatlimab led to a number of deals and R&D investments in ’20 and ’21. Unlike TIGIT, most of these deals have remained in place, the exception being Incyte who recently cut two LAG-3 candidates and returned them to Agenus and Merus. Regeneron’s fianlimab and Merck’s favezelimab are the two other LAG-3s with ongoing Ph3 trials. Aside from the KEYFORM-007 trial aforementioned, favezelimab also has a pivotal trial in Hodgkin’s Lymphoma and has studies in 7 different tumor types currently in phase 2, so there’s still plenty of opportunity to overcome this initial setback. On the other hand, fianlimab has more phase 3 trials underway (6) and is pursuing different tumor types than Merck – NSCLC and Melanoma. Notably, one of its recently initiated phase 3 trials in metastatic melanoma is a head-to-head study vs. Opdualag.  LAG-3 may still hold a few tricks up its sleeves, but the pressure is building to demonstrate that it can be more than a one-trick pony!
