What are the most common fallacies in risk management?
Past events can affect the present in many ways depending on innate personality or culture, and the habits, processes, and environment you create. A shock can make you more conservative in the future... or less afraid of things going the opposite direction from what you had hoped. A series of near misses may make you overconfident that the next time something will pull you back from the edge -- and as a result a potential crisis becomes more likely

What are the most common fallacies in risk management?
An over reliance on the idea that risk assessments can be objective and precisely quantifiable. In reality, risks are like fun house mirrors that change depending on your perspective and movements. Risks also are different for, say, well capitalized companies or not; experienced professionals or less so; flexible and resilient organizations vs not; and on whether decision makers with the power to act actually do so.

The Gray Rhino: How to Recognize and Act on the Obvious Dangers We Ignore
St Martin's Press
A "gray rhino" is a highly probable, high impact yet too-often neglected threat. Gray rhinos are not random surprises, but occur after a series of warnings and visible evidence. The bursting of the housing bubble in 2008, the devastating aftermath of Hurricane Katrina and other natural disasters, the new digital technologies that upended the media world, the fall of the Soviet Union...all were evident well in advance.

Drawing on her extensive background in policy formation and crisis management, as well as in-depth interviews with leaders from around the world, Michele Wucker shows in The Gray Rhino how to recognize and strategically counter looming high impact threats. Lockout: Why American Keeps Getting Immigration Wrong When Our Prosperity Depends on Getting It Right
PublicAffairs
for the first time ever, the world's best and brightest no longer see this country as the only destination of choice. In Lock Out, Michele Wucker documents the mistakes that led to our predicament today, and clarifies why it would be a catastrophic error of judgment, as well as a demonstration of a colossal lack of self-knowledge, if America attempted to turn its back on rest of the world and in so doing on the best of itself.
