Nice paper looking at 10 years incidence of DVT/PE. This is what I took from it: 1. incidence of PE was actually very low at 0.6%. This is, for the most part what we worry about. We have previously shown DVTs are common if you look for them so the incidence of DVTs is not a great yardstick. 2. Outpatient spine surgery was protective (get them up, get them home!) 3. Unbelievable work by Sidney Haitoff and the Mishe Health team. What once seemed like a distant pipe dream is now a reality. Sid’s passion and drive are infectious. His knowledge is deep and his vision is tightly focused. Having gotten to know him, I can attest to the work Sid has put in. This talk by emergency physician and AI practitioner Dr. Dan Buckland at Duke is an absolute must-listen for every clinician, hospital administrator, data scientist, start-up founder, venture capitalist, and anyone else interested in truly understanding the chasm between "AI idea" and "AI execution" in any health care related endeavor. Everyone is talking about #AI, but there’s still very limited application of #genAI tools in health care. Plenty of back-office use cases exist across revenue cycle, billing, prior authorizations – but it’s still very early days for clinical decision-making aids and other patient-facing genAI applications. BEFORE we see scaled deployment of these technologies in our clinical service environments, it’s essential we interrogate the potential risks and benefits to patient safety. I have during the last 24 years of my foray into Telehealth, have always maintained that irrespective of the fantabulous developments in Digital Health Healthcare should be patient centric. The beneficiary should be an evangelist par excellence. Social media, digital marketing, print media not withstanding the good old fashioned word of mouth recommendation still has a place. If only the term digital health or DH could be renamed CCDH (Compassionate, Caring Digital Health) the planet in the post DH era would be a still better. Bravo to Kevin Grumbach Deborah Cohen and Yalda Jabbarpour for this piece on #primarycare and the recent trends. "When Walmart, the corporation ranked number one on the Fortune 500 list, cannot achieve a successful business model for primary care, it reveals the root problems of thoroughly inadequate payment for primary care and underestimation of the work of primary care. Corporate and private equity investors—seeking extractive short-term financial returns and not committed to long-term investment—destabilize an already fragile primary care sector. Rapid start-ups and closures of primary care practices add turbulence to forces already buffeting communities dealing with declining access to primary care. The failing experiment of corporate, investor-owned primary care highlights the need for bold public policy to address primary care payment, infrastructure, and corporate ownership." #healthcare #healthcareonlinkedin

This is an interesting article because EHR companies, like Epic and Oracle's Cerner, have made siloing data a major part of their business model. Overall, I like EPIC’s ability to review records from other EPIC shops. It also has a nod to interoperability with its “Care Everywhere“ feature. However, this is often a disorganized and outdated collection of information. Using it requires a ninja level of both tech expertise and medical knowledge possessed mainly by younger physicians and chief residents. I am a fan (and advisor) of companies like Zus Health focused on user-friendly, comprehensive, and up-to-date interoperable health care data tools.

Shifting the financial model alone is not sufficient for the transition - the care delivery, workforce, and many other pieces of the ecosystem with which PCPs interact also need to adjust accordingly. Funding to allow for the startup costs of transformation must be made available.
