The (Healthcare) Ground Beneath Our Feet… This year it is likely that over $10 billion will be invested in the digital health sector – a robust amount, no doubt, but inconsequential when compared to the amount of capital tied up in healthcare real estate assets, which is estimated to be $1.2 trillion by Jones Lang Lasalle. Of that amount, approximately $790 billion is in the hospital sector while the remaining $490 billion is in medical office assets. It is estimated that there are 48.6k facilities with 3.4 billion square feet of healthcare real estate, 42% of which are owned by REITs and another 33% held by other private investment vehicles. Much of the digital health investment is meant to make the physical healthcare infrastructure more productive: greater operating efficiencies, greater patient throughput, more relevant personalized care models with better outcomes. As the healthcare system is being pushed to be more distributed to lower cost sites of care, accelerated now by the rapid proliferation of automation and AI capabilities, the potential impact on healthcare infrastructure assets will be profound. AI and Med Tech in Focus   AI System Developed to Identify Pathology in Tissues Engineers at Washington State University have developed an AI tool that uses images of human and animal tissue to detect abnormalities faster and more accurately than people. The AI algorithm was trained using images from past studies that involved kidney, testes, ovarian, prostate, breast and lymph node abnormalities. The new model detected disease in minutes—current processes take humans several hours and require specialized teams to check each other’s work. It was also found to be more accurate, sometimes detecting instances that humans had missed. The engineers designed the system to handle the processing load of the high-resolution gigapixel images by breaking them down into smaller tiles but still retaining the overall context of the tissue sample—akin to the zoom feature on a microscope or phone camera. Aside from getting diagnostic results more accurately and quickly, the new system also shows promise in epigenetics (the study of molecular processes that change gene behavior without changing underlying DNA). Epigenetic researchers can perform analyses and get results that used to take a year or more in just a couple of weeks. Sora AI just launched... I wanted to share my first text to video creation here, but as you can see, others beat me to it. In the words of The Californians (from SNL) - it's jammed! Until I can test drive it, here's some info: 1. Sora is a text-to-video service from OpenAI (ChatGPT folks). 2. You can also create videos from your images. 3. The videos are short: Up to 20 seconds 4. It's available to anyone - not just paid plans - from the Explore Feed. 5. ChatGPT Plus subscribers get up to 50 video generations per month at 720p resolution, with each video capped at five seconds. ChatGPT Pro subscribers get more 6. LLMs excel at summarizing and contextualizing existing literature, but cannot generate de novo medical evidence today. Someone broke down the estimated costs of a $15 Sweetgreen salad. We need these cost graphics for digital health to help future founders. Especially for tech enabled cos, the greatest cost is labor. Much of the "value created" is in curation and matching, which means the gross margins are reliant on labor arbitrage. Pay someone $X, but get paid $X+Y% per visit. The challenge with healthcare is that you often have to pick one of two business models: 1. specialized, low volume, high cost, low gross margin % 2. generalized, high volume, low cost, high gross margin % I'd argue founders should focus on gross dollars over the lifetime of the customer (like Jeff Bezos said) instead of gross margin %'s, but we'll leave that for another time. The challenge is that many tech enabled digital health cos have high cost of goods sold (labor) and moderately high product, design, and engineering budgets. We must build differentiated solutions for patients, clinicians, and clients to be out incumbents, however, we're realizing that companies are ultimately valued on the discounted value of future cash flows. And the high SG&A costs at most organizations might be inbalanced to the unit economics of the business. I'm not sure the math pencils out for everyone unfortunately. But I'm just a guy on the internet, would love to hear the thoughts from those smarter than I (you!). Comment below. --- p.s. Last week in San Diego, CEOs and VCs in the aging & longevity space got together to share their insights on anti-aging medicine. Here's a summary. 𝟰 𝗺𝗮𝗶𝗻 𝘁𝗼𝗽𝗶𝗰𝘀 𝘄𝗲𝗿𝗲 𝗰𝗼𝘃𝗲𝗿𝗲𝗱 -Types of outcomes investors want to see. -Changes in academic funding landscape -Regulatory landscape -Ethical considerations 𝗞𝗲𝘆 𝗧𝗮𝗸𝗲𝗮𝘄𝗮𝘆𝘀 Longevity is not about living longer. It's about increasing the number of years in which you are at your best and improving recovery from disease. Think of the resilience children have.  Aging is epigenetic. Changes in methylation patterns alter gene expression. This is not about telomeres or gene mutations anymore. First therapies to market may be AAV gene therapies. These therapies can restore methylation patterns to baseline and will focus on shared genes associated with aging and chronic disease. The 4 disease categories associated with aging are called the 4 horsemen. They are heart disease, diabetes, neurodegeneration, and cancer. Therapies addressing the above conditions represent a multi-trillion dollar market, and investors are looking to de-risk by putting money into clinical assets. Capital markets are slowly recovering with increase in IPOs and slight lowering of interest rates. CPI is still up and labor market is still in oversupply. Advise cautious optimism using the mantra "survive till 25'" Pre-clinical companies will have difficulty raising capital in this space. First-time CEOs should expect major challenges. NIH is putting more money into risky "moonshot" projects that encourage biotech partnerships, have commercialization plans, and can demonstrate path to market in 3-5 years. Projects are expected to have reasonable endpoints and broad applications. No more decades-long studies, and a slow move away from precision medicine. To increase accessibility and reduce cost, a larger specialized labor force is needed that can scale and administer gene therapies globally.
