For 11 consecutive years, reports like this one have found that the prices providers (hospitals, physicians, etc.) charge are the main factor driving Massachusetts’ high health care costs. While we value our hospitals and our health care institutions, it’s worth asking – what are we as taxpayers paying for, and at what cost? Individuals on Medicaid (taxpayer-funded) pay minimal to zero costs (~1/3 of the state’s population) and Medicare (taxpayer-funded) cost share is graduated based on income. But, for people with private health insurance (employer-sponsored health insurance, like Wellpoint members) health care premiums and out-of-pocket costs are taking up a significant –and growing– part of their income- between 20% to 40%. Perhaps it’s time to re-evaluate how taxpayer funds are being used to support health care and work with policymakers to reform reimbursements as means to help people stay healthy and out of hospitals.  In the meantime, below are some steps people can take to start understanding health care costs and stay healthy:   1. Think health, not sick care: eat healthy, exercise regularly, and stay active every day; 2. Take control of your health. Partner with your primary care team and be sure to get recommended preventive care and screenings; 3. Use primary care, urgent care, and community health centers for non-emergent care. These locations are the lower cost alternatives; 4. As an Emergency Department physician, UMass Memorial Health President and CEO Eric Dickson MD is adept at making tough decisions. And during our recent UMass Memorial Medical Group Physician Leadership Development Program, he emphasized that decision-making is not an individual sport. As leaders, we need to recognize that we don’t have all the answers ourselves. Instead, successful leaders surround themselves with experts who they can consult when challenges arise. It’s important to remember that our job as leaders is not to make decisions in silos, but rather to bring our trusted advisors together, hear their ideas and points of view, and work together to devise solutions. Sunday Morning Bathroom Read: Connecticut HB 5320 ( a/k/a the “Connecticut Public Defender and Risk Transfer Bill”) which recently passed the CT House of Representatives, creates a uniform application process and ELIGIBILITY criteria for financial assistance that requires not for profit hospitals to provide all medically necessary and emergency care services at the hospital’s full risk solely based upon an income level at 250% of the FPL ( $78,000 for a family of four) and without an asset test. Letterman Top 10 Market and CAA Implications: 1. The overall percentage of the US that earns an income level of 250% of the FPL is roughly 44%. I assume CT is similar; 2. Oscar and CIGNA recently announced that both will be leaving the CT small group market (https://lnkd.in/gmhH-gtd. ): 40% of the population effectively does not need insurance; 3. Plan design for CT employers moving forward: a) <250% of the FPL: MEC + CT Public Defender Plan; b) >251% of the FPL: RBR at the AGB% or ICHRA + fully funded HRA/HSA; c) Or depending upon the overall financial makeup of the group, ICHRA +CT Public Defender Plan; 4. Estate plan design (Risk Transfer): Ex):Business owner is diagnosed with cancer and has $1mm in cash and an income level of $500,000: Strategy: Slash income to 250% of the FPL and move this individual to the CT Public Defender Plan. There is a full risk transfer to facility and personal and business assets are unequivocally protected; 5. Joe lunch bucket (who has chronic back problems) is awarded a $100 a month raise which puts him above 250% of the FPL and on the company health plan. Maybe he declines the raise until his back gets better because the pay increase COSTS him more by being insured and paying premium; 6. Stop loss: For policies issued to plans in CT, I would include a provision that provides that claims are not subject to reimbursement in the event that an individual was eligible for financial assistance; 7. CAA Fiduciary Compliance: If the facility is "charging” FAP eligible patients its AGB percentage (40% of the CT market), shouldn’t everyone else get that rate? Medicaid, Medicare, Percentage of Walmart and CASH; 8. Claims data= medical, RX AND member financial information provided at the time of enrollment; 9. Office Linebacker: The Connecticut Health Systems Planning Unit; 10.Families that earn <250% of the FPL might just get a nice raise… . Happy Mother's Day to all of the mom's.

Who's going to buy Steward's remaining Massachusetts hospitals? Physicians can't own hospitals. Legislators have turned up the heat and regulatory scrutiny of PE and VC-backed healthcare (in large part because of Steward's bad apples). Smaller independent community hospitals (of which there are now few) are already struggling financially. Enter large, market dominant health systems? The state has made it clear it's not thrilled about Mass General Brigham owning more hospitals. (Instead, they've spun up outpatient centers in HNW areas and turned their attention to neighboring states). As a condition of approving its merger, Beth Israel Lahey Health agreed to a 7-year moratorium on cost increases among other concessions. That's set to expire in the next year or so. Do we really want more health system consolidation? (Studies would suggest the answer is "no" if you want lower prices, demonstrably better care and access, and fewer employed physicians). So, who's left? Are we willing to think a bit differently? Explore alternative options? Allow physicians or physician groups with business partners to make a go of it? (The horror, the horror). Can we admit that regulatory grandstanding has created as many, if not more, problems than it has solved? This is an opportunity to be a little bold. We shouldn't abide conflicts of interest or allow free reign for fraud and abuse. Oversight and transparency are paramount. But we can use a deft touch instead of a heavy hand. We can support innovation, not stifle it. This goes not just for Steward, but for efforts across the country to tackle hard problems by taking a different angle. Otherwise, get ready for more of the same. Same wolf, different clothing. #health #healthcare #medicine

I will continue to push back and say that criticizing the staffing mandate without mentioning how many Federal dollars are siphoned away to real estate and related parties does a great disservice to the discussion. That includes non-profit operations that have for-profit related parties and real estate. There are also thousands of quality facilities that staff at adequate levels and are financially viable. Behavior Analytic Colleagues!  ABA Progress Report for Health Professionals. An important collaboration orchestrated by the NYS Association for Behavior Analysis with the Developmental/Behavioral Pediatric community (as appropriately represented by Dennis Kuo, M.D., M.H.S, Chief of Division of Developmental and Behavioral Pediatrics at The University of Rochester Medical Center/Golisano Children’s Hospital) has resulted in a wonderful and simple form for Behavior Analysts to report to health professionals on therapeutic goals and progress for those with whom we share the care. NYS requires Behavior Analysts to report to “ABA prescribers” on an annual basis; while this form was developed to help NYS LBAs meet this licensure requirement, the basic structure and design of the form can serve the needs of Behavior Analysts everywhere. As a pediatrician -- and NYSABA member -- who works with both autistics and people with intellectual and developmental disabilities, this form provides an optimal framework to share summaries of goals, progress, and ambitions for therapy. This form can also serve as an important vehicle for education about behavior analysis; it is less about the details and more about the trends and aspirations – and could also help build better bridges between the child health and behavior analytic communities. We are now in the public comment period for the next round of cuts to the Medicare Provider Fee Schedule. The work that physicians and other clinicians do continues to be discounted as part of the goal of budget neutrality. Dr Rob Zipper explains the unforeseen consequence of these cuts and the lack of an inflation index which is approximately a 25% reduction physician salaries over the last 20 years. Many physicians and group practices have looked to hospitals and health systems to make up for part of that gap, however, that has been difficult in recent years. In a country where we want to continue to recruit the best in the brightest into healthcare careers, this problem needs to be addressed. Last year there was proposed legislation to add a Medicare inflation index to the clinician payment structure, an approach already used for hospitals. That didn’t pass so we are once again facing additional cuts without any inflation adjustments. The legislature needs to evaluate the opportunity to eliminate additional cuts in payment to physicians for clinical care of patients and to tie the Medicare Provider Fee Schedule to an appropriate inflation index.
