I had the chance to speak with Joshua Parry, MD, MS, of DENVER HEALTH MEDICAL CENTER, about results presented at the Orthopaedic Trauma Association Annual Meeting that showed patients with stress-positive lateral compression type 1 pelvic ring injuries who had #fixation were more likely to ambulate without assists at last follow-up and have a lower mortality rate vs. those who did not have surgery. “Right now, the standard of care for these injuries is nonoperative treatment, but I do think some patients, similar to hip #fractures, warrant early fixation and mobilization and that may prevent mortality,” Parry said. OUR NEW YEAR RESOLUTION: ELEVATING PCOS CARE TOGETHER https://lnkd.in/gy5SWQjX My heartfelt purpose is helping primary care providers to deliver expert care to women with PCOS in their communities. At the University of California, Davis Medical Center, I developed the PCOS program, treating thousands of women who had to travel long distances. My experience underscored the necessity of community-based care for over seven million PCOS patients in the USA. I strongly believe primary care physicians can provide expert PCOS care. Diagnostic tests for PCOS are readily available, and primary care doctors frequently prescribe treatments like birth control pills, metformin, and spironolactone for other disorders already. Unfortunately, the current medical care model and education do not prepare primary care physicians to provide the optimal care for PCOS patients. The current models lack a cohesive approach, dividing care among various specialties such as obstetrics, gynecology, dermatology, and endocrinology. To bridge this educational gap, I am launching the "Advancing PCOS Care: Empowering Health Care Professionals" course for primary care providers. This course offers comprehensive knowledge and skills, along with mini-coaching sessions to enhance competence, confidence, job satisfaction and career growth. Insightful article Our healthcare system does not support pediatric care as it should. Drs. Satyan Lakshminrusimha, David Lubarsky, @Rhae Gamber & @Sherin Devaskar nicely highlight the significant issues associated with the current state of #Pediatric #Healthcare, focusing on reimbursement which is a cause for many downstream issues. It is an article filled with solid and important information and a call to action. https://lnkd.in/g8ZNkYQT More than half of babies less than 34 weeks, the most fragile and care complex newborns, are covered by Medicaid across the U.S. There is disparity as more than half of Black, Hispanic, native Hawaiian/Pacific Islander, American Indian, Alaskan native babies covered by Medicaid in the NICUs across the U.S.. Fee for service (FFS) is the most common form of reimbursement for providing NICU (many have rightfully so pointed out that this is a perverse payment model). In many cases FFS reimbursement rates do not cover the cost of an NICU hospitalization and are below payment rates for comparable Medicare services. The authors highlight the impact that low reimbursement for nonprocedural, cognitive-based specialties is having on Pediatric subspecialty access-- a trend that appears to be worsening with recent data demonstrating a decrease in residents matching in various pediatric subspecialties. For the recent pediatric subspecialty match in 2023, there were unfilled fellowship positions. 56% genetics, 47% nephrology, 52% ID (among many others) slots were unfilled. That is an alarming percent for specialties that fill a very necessary role if we are to provide the best care possible to newborns and children. I do disagree with one of their statements that "supply-demand will increase pediatric subspecialty salaries over time." I do not think that physician salaries lies within a truly competitive market that would follow the laws of supply and demand. We also need to be aware of the fact that Pediatric care access, both inpatient & outpatient, is decreasing over the past decade and really accelerated during the Pandemic when many Pediatric beds were converted to adult beds which have not transitioned back because more $ can be made from a adult occupying a bed than a child. Many community hospitals have closed their small, but important Pediatric units. Rural & critical access hospitals have closed with many teetering. Access to Pediatric primary care is becoming more limited, especially in those unfortunate patients with Medicaid coverage (which is a majority)-- creating widening and disheartening Pediatric Care deserts across the U.S. Their take-home message: "Increased Medicaid reimbursement is critical to the sustainability of high-quality pediatric subspecialty workforce that is family-centered, timely, effective, efficient, equitable." #UsingWhatWeHaveBetter

Problem: This type of junk food is pervasive throughout hospitals and medical schools across America. New data from the OlympiA clinical trial presented at the 2024 San Antonio Breast Cancer Symposium (SABCS) show that the PARP inhibitor olaparib (Lynparza) significantly improves survival in patients with high-risk, BRCA-positive, HER2-negative early breast cancer. The findings, after a median follow-up of 6.1 years, further support the drug’s use as an adjuvant therapy after standard treatments like chemotherapy, surgery, and radiation. “The OlympiA trial examines adding one year of the oral PARP inhibitor olaparib after completion of standard treatment for higher risk breast cancer in individuals with pathogenic germline BRCA variants,” said Judy E. Garber, MD, chief of the division of cancer genetics and prevention at Dana-Farber Cancer Institute and one of the principal investigators of the OlympiA trial. “Data from the third prespecified interim analysis presented here provides further support for olaparib’s benefits in patients with high-risk, HER2-negative breast cancer with germline mutations in BRCA1 or BRCA2.”

A new study led by The University of Texas Health Science Center at San Antonio (UT Health San Antonio) is shedding light on a novel role of breast cancer gene 1 (BRCA1) in tumor suppression. Individuals with inherited mutations in BRCA1 are predisposed to breast, ovarian and other cancers. BRCA1 helps prevent cancer by repairing damaged DNA, but how it does so is yet to be determined. The damage in question is the DNA double-strand break, where both strands of DNA are broken and can lead to cancer if not repaired or if repaired inaccurately. The new research led by UT Health San Antonio shows that BRCA1 not only pushes a DNA break toward an accurate DNA repair mechanism called homologous recombination (HR) but also promotes subsequent steps by spurring the activity of "end resection enzymes" that process DNA ends to prepare them for HR. "Our biochemical analysis with purified BRCA1 protein helps illuminate its role in DNA end processing," said Patrick Sung, DPhil, associate dean for research at UT Health San Antonio and director of its Greehey Children's Cancer Research Institute. He is a senior author of the new study titled, "Promotion of DNA end resection by BRCA1-BARD1 in homologous recombination," published Sept. 11 in the journal Nature.
