States are employing various means of reaching diverse and needy families through single touch points and coalitions with other entities such as faith-based organizations. I was happy to see that academia was mentioned at least in one example as part of coalition building. Also mentioned are recruitment strategies for building respite workforce. Some states are hitting all the right buttons and these should serve as models for other states, and also for expanding help to other areas beyond respite. The best way you can ensure innovative programming at the state and federal level is to let your legislators know that these are priorities. Today, KFF launched the Health Misinformation Monitor as part of our new Health Misinformation and Trust Initiative. It tracks the spread of health misinformation in the U.S. with a goal of helping you stay up to date and strengthening efforts to counter misinformation. The last time Congress passed a comprehensive disaster package was in December 2022. The short-term Federal disaster response has been funded through FEMA’s Disaster Relief Fund, which supports Stafford Act authorities and is in need of additional funding. What the Stafford Act does not account for is long-term recovery. Health centers need the Administration and Congress to work together to ensure any upcoming disaster supplemental funding package includes sufficient resources to help health centers keep their doors open and continue serving their patients.

A recent report has unsurprising findings about the challenges in accessing and assessing Medicaid HCBS, and includes a necessary call for improving how Medicaid HCBS is delivered! "A maze of federal legal authorities has resulted in inconsistent and inequitable access to [long-term] care among [Medicaid] beneficiaries and across geographies. Anger, disappointment, disbelief, fear, stress, and, anxiety, is the weathering Black birthing bodies treated in healthcare systems encounter each day. This publication paired with the 9/10/24 New York Times article exposing the findings of one of the largest studies that confirmed the drivers of Obstetric Racism in NJ played out through unnecessary C-sections on Black pregnant people to fill operating rooms, is a reminder of just how much oppression, discrimination, and dehumanization the Black reproductive journey faces. The dots are connecting and it is no longer hidden in plain sight. Black bodies are most profitable when they have no agency, autonomy, or control over their bodies. If the 25% of forced elective C-sections given to Black birthing people in NJ had Doulas, would there be a profit margin for the healthcare systems and providers that operated the 68 hospitals in the study?? In the 1600’s one Black body was worth an extra 10 acres of land to fuel the New Jersey economy. Today in the healthcare system what’s the profit margin of one Black womb in New Jersey???? Commodification of Black bodies in New Jersey has not stopped clearly. The $2,246 funeral expenses cap or the $524 for a Medicaid recipient in NJ to be handed back to their family as a container of ashes may be much cheaper than just sustaining Community Doulas, which evidence based research asserts can reduce maternal mortality and morbidity, especially where disparities are in the southern region of the state. (Dreaa Austin and others should still be alive). Can a pregnant person with Medicaid in Salem, Atlantic City, Bridgeton, Camden, Pleasantville, or Millville even find a Community Doula? When will this resolve? For every family that does not have access to evidence based resources and supports meant to improve birth outcomes, it is also a family that should consider a mortuary pre-need. Why? Because Black women are STILL 7x more likely to die in NJ in comparison to their White counterparts. NJ is still 29th in Maternal Mortality outcomes. Moving from 47th over a year ago was not a victory. We moved simply because other states like Louisiana, Mississippi, Tennessee, and others did worse. The more information and studies that emerges the more unsafe and less empowered some people in disparate communities feel. Kaiser Permanente is actively engaged in the work of screening our members for social vulnerabilities like housing- and food-insecurity and referring people who need support to community partners that can help with these essentials. While we hear frequently how life-changing this support is for our members, it’s encouraging to also see that addressing social needs in the heath care setting can also reduce health care costs and avoidable utilization. The latest evaluation report from the Centers for Medicare & Medicaid Services Accountable Health Communities Model shows that linking health care and community organizations together to help Medicare and Medicaid beneficiaries with core social factors reduced health care costs, emergency department visits, and hospital admissions. Yes yes and more yes. Time to free public health from health care and the traditional ways that “health” is funded and valued. They talk innovation and social determinants but they place value on immediate and direct clinical outcomes when the science shows that that’s not how it works. If you want to address SDOH then you have to put your money where your mouth is. And let us address the things that matter without holding the checkbook over our heads that’s tied to healthcare savings.

To keep up momentum in the fight for health equity, we need to celebrate wins. That’s why I wrote about some good news in the health policy space for RWJF’s blog. I wanted to share recent efforts by RWJF to make healthcare more inclusive across the U.S. We submitted public comments on several federal rules to help make sure they were written in a way that best serves the public. My "only good news on Friday" comes from New York City and beyond, where 32BJ SEIU and Building Service 32BJ Benefit Funds have been educating and mobilizing in NYC (the belly of the beast of powerful hospitals) to lower hospital prices and put more money in workers' pay checks. Their "Price of Power: Confronting the Hospital Affordability Crisis" brought powerhouse resisters together to build power for patients, workers, and employers. Site neutral payment, antitrust enforcement, transparent prices, and direct contracting are top agenda items. Zack Cooper showed how hospital consolidation is slowing local job growth and depressing wages of working people. Ryan Danks of USDOJ Antitrust Division described how consolidation, non-competes, and anti-competitive contracting inhibit health care innovation. Newark, NJ Mayor Ras Baraka took the reins to negotiate better deals with hospitals for city workers. Courageous NYC Councilwomen are taking on hospital affordability through transparency and public accountability. Public employers like Miami-Dade Schools are demanding that local hospitals be better civic leaders or else they won't be included in plan networks. How might health companies move from supplying health to preventing it? Is there a model for bolstering population health by catalyzing community building? The Greater Rochester Health Foundation offers an example of what is possible. It aims to boost heath outcomes by focusing on enhancing #community capacity to produce #health in both urban and rural #neighborhoods. Importantly, it takes long-term view (10 plus years) and does not impose specific predefined outputs or outcomes. Instead, the Neighborhood Health Improvement Initiative supports resident groups and community animators to reweave the neighborhood social fabric--increasing collective efficacy and social support networks in the process. The foundation explains why it adopts an "asset-based, grassroots efforts to improve the physical, social, and economic environments of neighborhoods" in the Greater Rochester area as follows: "Our daily lives and the neighborhoods in which we live them—where we raise our families, work, and play—along with our personal health habits, affect health in countless and complex ways. Some neighborhoods support health and healthy behaviors better than others. In healthy neighborhoods, we feel safe walking outside, can access green space for recreation and physical activity, and we can purchase and eat healthy, affordable food. Healthy neighborhoods are free of abandoned housing that attracts crime and are places with trusted neighbors to turn to when in need." Deborah Puntenney offers more detail: "an inner-city community....The people who are involved are neighborhood residents, individuals, families. They’ve come together in block clubs, they have a neighborhood council. But these are informal groups, none of these are 501(c)3s except for the organization that actually received the grant, which is a CDC.... They finished their asset map and then they met as a community and they had identified several issues that they wanted to work on. Healthy eating, healthy exercise, and doing something about the open air drug market that had plagued their community for about 30 years.... Today when you go through the neighborhood, people comment all the time, this neighborhood group has managed to build community gardens, it has a developed a park and playground, it has completed a major trail going all the way through Rochester and right through the neighborhood. It has launched educational opportunities, it’s got numerous activities for seniors, for youth. It has developed major partnerships, and by that I mean between neighborhood residents, the mayor, the police, the courts, the judges, the district attorney, anybody you can think of they are partnering with. There’s been massive community change in this area.
