Hot take on a lot of early-stage digital health companies out there: You're not a tech company. Most tech-enabled care companies I've seen over the past two years are only tech-enabled in the sense that they use telehealth as their delivery mechanism for care. There are exceptions to this, of course - companies who build computer vision and applied AI tools to better the doctor-patient experience, companies who build tooling around risk scoring and stratification to enable more personalized medicine, companies who build truly novel intakes to better route patients to the appropriate care, etc. These companies have real technology chops and, if they sell it right to the market, can get credit for this from investors. But if you're truly a telehealth provider, you need to realize now that the goal post has shifted dramatically from 2021 and you're going to be valued at maturity at something like 3x EBITDA. That's profit. Not revenue. Throw a revenue multiple out the window. That's not the world we live in if you're a virtual doctor's office. Think of how that impacts venture math. If you're raising a seed round at $15M post-money (or $12M pre-money), it means you'll have to generate something like $150-200M in annual EBITDA to be a fund returner for your seed investors. Maybe even more with dilution, since telehealth businesses are not traditionally capital efficient and will likely need to raise material additional capital to reach that scale. You need to be ridiculously profitable (probably on $800M-$1B in annual revenue since you're likely going to be a 40% gross margin business). Here's the good news: you can set yourself up well (and probably be a contrarian founder relative to your peers), by talking about EBITDA growth and how you can grow into a fund-returning outcome based on realistic mature market comps. This will stand out for the investors you're pitching, even at the early stage. This is how you can beat the malaise of the tech-enabled care market. Focus on the right metrics and the right type of growth. You don't have to have all the answers (no founder does), but you need to make sure the ship is headed in the right direction. A focus on profitability for "tech-enabled care" (*ahem* telehealth) businesses is a good first step.

What happened in mental health last week? ⏺ Anise Health raised $3 million to deliver culturally-responsive mental health care for the Asian American community ⏺ Goldman Sachs acquired autism therapy provider Center for Social Dynamics, a care provider to over 4,700 clients across 20 locations. ⏺ Lyra Health CEO David Ebersman announced that he will be stepping back from his role to care for his mental health. Ebersman will take on a new role as Board Chairman. ⏺ Substance use disorder (SUD) treatment provider BrightView is closing nine facilities in Massachusetts and Arizona citing challenging operating environments. ⏺ California introduced a bill that would require mental health warning labels on social media sites. It would be the first U.S. state to do so if the bill is passed. ⏺ The Australian Medical Association issued a position statement warning that chronic underfunding and increasingly complex patient presentations are pushing the mental health system to a crisis point. ⏺ Ukrainian startup Healthy Mind raised $1 million to launch an AI platform for mental health in the workplace. ⏺ Indian mental health platform GoodLives raised $129,000 in a pre-seed round, offering AI-driven self-directed exercises alongside expert-led therapy sessions. ⏺ We (The Hemingway Report) launched our 2024 founder survey. The (Healthcare) Ground Beneath Our Feet… This year it is likely that over $10 billion will be invested in the digital health sector – a robust amount, no doubt, but inconsequential when compared to the amount of capital tied up in healthcare real estate assets, which is estimated to be $1.2 trillion by Jones Lang Lasalle. Of that amount, approximately $790 billion is in the hospital sector while the remaining $490 billion is in medical office assets. It is estimated that there are 48.6k facilities with 3.4 billion square feet of healthcare real estate, 42% of which are owned by REITs and another 33% held by other private investment vehicles. Much of the digital health investment is meant to make the physical healthcare infrastructure more productive: greater operating efficiencies, greater patient throughput, more relevant personalized care models with better outcomes. As the healthcare system is being pushed to be more distributed to lower cost sites of care, accelerated now by the rapid proliferation of automation and AI capabilities, the potential impact on healthcare infrastructure assets will be profound. One of my proudest moments as an investor occurred today as Alaffia Health announced its series A because it shows how the Aperture® Venture Capital vision of multi-level, multi-generational #impactinvesting is succeeding in the marketplace. As part of Forum Ventures’ investment thesis in healthcare, Naomi Goez and I spent a lot of time looking at the healthtech landscape to find the fastest-growing early-stage (Pre-Seed to Series A) startups building in three categories:    1. Healthcare staff management  2. Evolution of digital health 3. Operational productivity in healthcare systems Our objective is to gain a deep understanding of the market dynamics: who is building what, what companies are having success and why, where the gaps and opportunities are, and determining the optimal investment areas for achieving maximum impact and returns. We built a market map based on this research and shared it a few months ago. We’ve since had a number of startups request to be added – too many to add to our static visual. So the team built a new online version that can house hundreds of companies easily and continually be updated live as more companies are discovered. I recently spoke with an executive from one of the largest healthcare companies in the country.   When I asked them what are the highest priority problems they're concentrating on right now, unsurprisingly, 1️⃣ controlling costs around GLP1s came up first. But surprisingly, 2️⃣ the intersection of health and climate change came up second! It is clear that more healthcare leaders are beginning to think about (and respond to!) the deep interdependence of human and environmental health.   🗣 While my climate friends have long emphasized that "climate is a horizontal," and "there's no such thing as a climate company; all companies will have to adapt to climate change", I'm energized to hear more of these conversations beginning in healthcare.   😷 We already know that wildfires and air pollution are driving increases in respiratory and chronic illnesses, that heat waves and natural disasters are putting more lives at risk, and that rising temperatures and shifting rainfall patterns are enabling the spread of infectious diseases.   As we invest in downstream solutions that support patients suffering from respiratory diseases, cardiovascular diseases, anxiety, depression, and general gaps in gaps in health equity, we're also thinking about holistic, upstream factors, like the effects of climate change.   ⚕ 🤝 🌎 How should we evolve our traditional notions of preventive health to address these emerging challenges? ⚕ 🤝 🌎 How can health-first organizations play their part when it comes to the climate crisis? ⚕ 🤝 🌎 What are the innovative ideas you've seen around climate, sustainability, and human health?  💡 This is both a personal area of interest and something I'm hoping we get to explore more at Inflect! Today was a big win for consumer health, with Flo Health Inc.'s valuation at $1 billion. We're hearing talk about the market becoming frothy again, but I’d argue—without seeing the detailed financials—that this valuation could be entirely warranted. Flo has built an impressive business, with 5 million paid subscribers and 70 million monthly active users. At $60 per year for users to access the product, this business has hundreds of millions in revenue and plenty of room to grow. It's increasingly clear to me that innovators focused on healthcare have been wildly underestimating consumer business models for years, believing that patients would only opt for solutions covered by their health plans and prescribed or recommended by their doctors. That thesis needs updating, particularly given high deductible plans and soaring co-pays. Innovators building solutions that users genuinely love will win - even in healthcare. These businesses also have the potential to move into B2B channels, which is far easier to do when you have an engaged, loyal user base. I expect we’ll see many more consumer health businesses “pop” in the years to come, further proving that consumers are ready and willing to pay for such solutions. We're watching closely at Cherish Kudos to the team at Flo for this impressive milestone. A really great and well timed updated highlights on European Femtech (health tech and female health) referencing the H2 2024 trailblazing Sie Ventures report. Thanks for flagging Ida Tin and sharing as additive data to my "Femtech series". - report focuses predominantly on the "femtech" market size and opportunity, but brings in some broader "female health" data points (some only as at H2 2023). - Also lists the recent momentum in regulatory/governmental initiatives in the EU that focus on female health (the US has seen the same trend). -Luckily we have seen continued momentum of late stage fundings since this dataset/H2 2024. How did technology/femtech trigger female health momentum? - technology "femtech" has been the driver of this regulatory momentum. - It has taken digital biomarkers to focus in on how female cycles affect disease characteristics, drug efficacy and dosing and that "pain" has been normalised for female interventions - Now translational therapeutics focused pharmaceutical companies are partnering with femtech firms and wearables are providing the diagnostics! - We are at an intersection! What has biotech/translational therapeutics struggled with? - Biotech and drug development has therapeutic trends around well known potential drug targets i.e. translational medicine. - The general VC rule of "large market" holds, or a "niche/orphan disease" with lower BTE to get a drug to market faster - Even female oncology indications such as breast or ovarian cancer have been heavily developed for - with mixed success. - Other female diseases have seen no innovation for decades (e.g. endometriosis) - Female biased or female differentiated diseases, have therapeutic regimens dosed for men with seldom sub-population efficacy analysis (or parity representation) in clinical trials. BIAS v. Market Opportunity/Category leadership -We still need to address investment bias such as mirroring in male dominated investment firms with predominantly male deal funnels -Terminology bias i.e. "femtech" can create the perception that gender lens investing is impact or charitable - But series C funded unicorns seen over the last quarter have provided exits for early stage VCs validating the commercial market potential is! I've seen countless startups stumble on regulatory strategy - it's painful, expensive, and often avoidable. The seminar shared by David Pudwill and Geetha Rao hosted by UCSF Innovation Ventures, absolutely nailed what matters most. Here are my key takeaways that every founder should know: 1. Understand your scope - this is where it all starts or stops You've got to nail this foundation: --Map out ALL potential clinical uses (yes, all of them) --Find your "regulatory siblings" (similar approved devices) --Get crystal clear on your impact level: 1)Wellness management (easiest route) 2)Screening/monitoring (middle ground) 3)Diagnostics/therapy (time to bring your A-game) 2. Pick your lane: regulatory pathways from "Easy" to "Buckle Up" Here's what you're looking at: --510(k) Exempt: The startup favorite (for good reason!) --510(k): Tried and true path --De Novo: The road less traveled (but sometimes necessary) --PMA: The regulatory marathon (pack your endurance) 3. Strategic roadmap: This part separates winners from the pack Smart execution is everything: --Start minimal, but make it smart --Grow strategically (think chess, not checkers) --Nail the execution sequence: Define → Design → Test → Produce → Sell Pro tips from the investor seat - know your audience and speak their language: --Investors need that solid IP story --Clinicians want real-world outcomes --Regulators love familiar territory (use it) --Claims strategy (this is crucial) Three rules to live by: --Start small (seriously, resist the temptation) --Expand methodically (no shortcuts) --Document everything (future you will thank you) Why this matters to investors - here's the bottom line: "Your regulatory strategy IS your market strategy. Awesome report from Rock Health (in the comments) about H124 digital health funding. 📈 Despite broader economic challenges, the digital health sector, including mental health, has shown resilience, raising $5.7B across 266 deals. There is a strong Series A pipeline, tapering off of unlabeled funding rounds and 3 public exits in Q2 2024 indicating a revival in the exit market and a return to more sustainable funding patterns​. Key insights about the mental health market: 🧠 Mental health continues to be the leading clinical indication in digital health funding, securing $682 million in H1 2024. This consistent top ranking highlights the ongoing investor interest and the market's recognition of the critical need for mental health solutions. 🤖 The focus on mental health is driven by a combination of increased awareness, the rising demand for accessible mental health services, and the integration of tech solutions...🙄 In fact, AI has become a significant factor in digital health investments, with 34% of the total sector funding going to AI-startups. What happened in mental health last week? ⏺ KHP Ventures announced the Innovations in Mental Health Fund, a £20m fund to invest in mental health startups ⏺ Amae Health raised $6m from Cedars-Sinai for psychiatrist-led integrated care and wraparound mental health services for severe mental illness ⏺ Warsaw-based Britenet, received a £1m grant from the EIT - European Institute of Innovation and Technology for Moodmon, an AI driven tool for management and treatment of chronic affective disorders ⏺ Pulvinar Neuro received FDA breakthrough device designation for its closed-loop transcranial alternating current stimulation (tACS) to treat major depressive disorder ⏺ Czech startup Elin.ai closed a €1m pre-seed round for a mental health app for Gen Z and Gen alpha users ⏺ MindMed announced that their first patient has been dosed in their phase 3 study for MM120, an optimised form of LSD for generalised anxiety disorder after being granted ‘breakthrough therapy’ status by the FDA in March ⏺ Isar Mazer from Tacklit and Jacinta Pollard from Caraniche published an intersting article on how to digitally transform mental health services ⏺ Autism therapy provider Proven Behavior Solutions, LLC acquired Prism Autism Education & Consultation Education and Consultation, expanding into a second state. ⏺ Redesign Health raised a $175m fund to back health-tech startups.
