Epic Systems’ recent move to back the TEFCA medical records exchange is interesting. On the surface, it’s all about creating a seamless flow of patient data across hospitals, QHINs, and all relevant stakeholders. But let’s be real—data is the oil of the new economy, and interoperability is the pipeline that delivers this precious resource to all the right stakeholders. By aligning with TEFCA, Epic might just be positioning itself as the refinery. All in all, this is a welcoming move by Epic Systems—especially if it really is as straightforward as they make it sound. But given the lack of details, one has to wonder if there are any hidden catches. After all, when it comes to something as valuable as healthcare data, it’s wise to look out for the fine print. Interesting perspective on #TechnologicalDebt in health care by Dave Lamar focused on the provider market, but we seem the same issues within state depts and payers. While good data governance practices are key, there must be a better way to solve this common - if not universal - challenge in health care, if only by offloading some of the administrative responsibilities that require the individual management and administration of no-value-add administrative functions. #healthdata

Eric’s article below triggered a flurry of thoughts for me so it inspired me to repost and offer a more complete opinion. So here goes nothing :-) … I love his use of Swift’s (no, not that one!) inspired satire and the element of shock to effectively grab our attention. I found myself gasping and clutching my pearls at that very bold 4th sentence even though I knew he was leading us to a parallel universe immediately after reading his introductory sentence. As someone whose English is not the first language, I reverted back to “wait, am I not understanding this correctly?” when I read about the excessive allocation of funds. 🤨 This was me when I read that. It was like whaaat??? Still, by the time I hit the bold pronouncement that Supply Chains should be eliminated, my heart skipped a beat. Let’s face it.. #HealthcareSupplyChain is generally not naturally the 1st, 2nd, nor 3rd recipient of the bounties and operational strategic improvements that are laid out across healthcare organizations. Certainly, there are exceptions but the reality is that even though many have physically moved from the proverbial basement (next to the morgue) to the vicinity of the Boardroom, as an industry, we are still shackled with the characterization that what we do is simply #purchasing. Nothing is more baffling to still hear many casually refer to the complexity of the Healthcare Supply Chain engine as the act of Purchasing or Receiving. While those 2 functions are important building blocks in our ability to show up, delight, and deliver ( no puns intended), we have evolved into an incredibly complex and integrated network within an even more sophisticated and ever expansive system. Very few can happen without the intervention or facilitation from our Supply Chain. As a transplant #NewOrleanian, I view us as #glitter! We are everywhere, we stick to you, you see us from afar, we brighten your life, and you cannot get rid of us! ☺️ So, Eric’s characterization of the complexity of our profession and the impact we carry across the entire continuum of care (& beyond), is gratifyingly accurate. I want to specifically call out the plea to heavily #invest and anchor Supply Chain’s expanded clinical, financial, operational, & strategic impact in acute & non-acute settings. We must maintain a strong focus on qualitative, technological, & data enablement. I also appreciate Eric’s point to leverage this as a profession deserving of title & scope expansion, talent development, strategic structure alignment, & resource optimization. I would also add to afford credible consideration to an uncluttered path to the Boardroom. It’s been said, and I agree, that the next CEOs will come from Supply Chain. How can #Medicaid policymakers advance the adoption of value-based arrangements by Federally Qualified Health Centers? 1.     Through the Return of Shared Savings to #FQHCS. – Participating FQHC networks should be eligible to share in a substantial portion (50% or more) of the savings realized when the actual total costs incurred for their attributed population are below the benchmark or goal set for average Total Cost Of Care (TCOC). 2.     Adjust the Payment Model for the FQHC Patient Population. – The benchmark costs used to set rates should be based on the average #TCOC in the market, or the average amount the Managed Care Organization gets paid per enrollee, rather than using the FQHC networks’ own historical low reimbursement as a starting point.
