Value based care (VBC) 1.0 language was heavily oriented to language like "disruption". This language was like a punch in the mouth and was effective at building awareness of new opportunity - but disruptive punches to the jaw don't sound like a very attractive end state. It sounds pretty painful, and maybe that is one reason why VBC adoption has stagnated. VBC 2.0 needs to be about integration and partnership so that we can start to bring real value to patients and the provider organizations who serve them - and do that at scale. The pre-eminent leader in health plan clinical guidelines coming together with the most innovative AI provider of utilization management technology is a perfect example of this new paradigm. Kudos to the MCG Health and Cohere Health teams taking a leadership role to make the health care system work better for all.

Argonaut's 2024 "FHIR Write" initiative launches today, focused on continuous glucose monitoring! We're streamlining standards-based integration of CGM data into EHRs to support collaborative diabetes management. Huge thanks to Redox, Rimidi, Rhapsody, Sensotrend, and Tidepool for helping me and Brett Marquard come up to speed ahead of the project launch. I also want to share my personal pre-launch exploration to learn about CGM: 1. Got a CGM prescription and began tracking my blood sugar. 2. Mapped my CGM readings to FHIR Observation and Device resources. 3. Calculated key metrics like "time in range" as FHIR Observations 4. Created a visualization PDF based on the Ambulatory Glucose Profile, and wrapped this in a FHIR DiagnosticReport 5. Published same visualization as an interactive web view 6. Packaged all this up in a SMART Health Link for secure, structured data sharing + built-in visualization This SMART Health Link represents my past 120 days of glucose history, updated automatically and covering all core data elements described in the Diabetes Technology Society (DTS)'s iCoDE data set: ** Josh's glucose SHLink: https://lnkd.in/g2TQb5tN ** Anyone with the SHLink can access the structured FHIR resources behind the visualization. In fact, the viewer itself is a SHLink client that knows how to render the data it finds. This is an interesting article because EHR companies, like Epic and Oracle's Cerner, have made siloing data a major part of their business model. Overall, I like EPIC’s ability to review records from other EPIC shops. It also has a nod to interoperability with its “Care Everywhere“ feature. However, this is often a disorganized and outdated collection of information. Using it requires a ninja level of both tech expertise and medical knowledge possessed mainly by younger physicians and chief residents. I am a fan (and advisor) of companies like Zus Health focused on user-friendly, comprehensive, and up-to-date interoperable health care data tools.

Large Healthcare Technology Consulting Firms vs. Smaller Specialized Consultancies As I have discussed in previous posts, the field of technology consulting, and healthcare technology consulting in particular, is currently experiencing a downturn. Year-over-year revenues for healthcare consulting are declining, and this situation is especially challenging for smaller firms compared to their larger counterparts. Larger consultancies have the advantage of deeper cash reserves, allowing them to endure economic downturns with greater ease than smaller boutique firms. However, it is important to remember the reasons you have chosen to work with smaller firms in the past. Their specialized expertise in areas such as Revenue Cycle Management, Acute Care, or Clinical Systems Interoperability has been directly relevant to the specific issues you have encountered with your systems. Larger firms often apply generic solutions that may not be the best fit for your unique needs. Therefore, it is crucial to maintain relationships with your boutique vendors and support them whenever possible. If these specialized firms disappear, you might find yourself without the precise expertise you need when challenges arise.

What are the most significant obstacles to using electronic health records (EHRs)? Klas Research asked 33,000 clinicians across over 300 healthcare organizations this question and compiled their answers in the 'EHR Interoperability 2024' report. When asked specifically about their experiences with patient data from external organizations, 47% mentioned the lack of proper integrations to access EHRs. I've done many maturity assessments and capability models over the years especially at the intersection of emerging technology and Business Architecture to improve operations. At their best these assessments helped inform priorities and roadmaps and were one step on a business architecture journey. At their worst it was astrology for business leaders. "High my name is Glen and our CMM score is 2 but we are learning quickly". This pattern seems to offer a lot of opportunity for productization of services. How are others seeing this activity as a value driver in their organizations?

Now that our HIT industry has spent nearly 40 years figuring out how to effectively pass data between sites, settings, and institutions of care (and while that journey isn't completely over, we're well on our way), we can finally turn and look farther down the road and consider what's next. In my view, advancing process consistency, agility, and change is that horizon. The BPM community is focused on that challenge, and having recently transitioned into HL7 as HL7 BPM, is squarely trying to take this on. With today's press release, HL7 is truly open-for-business in this space. Thanks to the OMG for nurturing this work along and for HL7 for having made our next home for this effort.
