Patients struggle to afford medications, leading to unnecessary ER visits As an emergency physician, I frequently encounter patients with uncontrolled hypertension or diabetes. These chronic conditions, which should be manageable, often bring patients to the ER because they can't afford their medications. During my shifts, I ask patients if they are taking their prescribed medications. Many respond, "No, I can't afford them." When I inquire if they've discussed this with their primary care physician, they often say they haven't. This lack of communication stems from discomfort discussing financial issues or a lack of trust in their patient-doctor relationship. Consequently, patients leave with prescriptions they can't fill, leading to worsening conditions and repeated ER visits. This cycle not only deteriorates their health but also results in large medical bills for preventable emergencies. Many of Medicare's COVID-era telehealth flexibilities are set to expire at the end of 2024, including coverage of audio-only telehealth visits, which may improve access for individuals with limited internet connectivity. In a new JAMA Health Forum paper, we examine recent trends in audio-only telehealth use, and find that certain groups may be disproportionately affected, including low-income patients, people with multiple chronic conditions, and members of racial/ethnic minority groups.

Studies have consistently shown that prolonged ICU stays are associated with higher mortality, worse long-term survival, and poorer quality of life compared to patients with shorter ICU stays. A systematic review and meta-analysis found that protocolized family support interventions significantly reduced ICU length of stay. Sunday Morning Bathroom Read: Reasons to be skeptical of government control of all health care: 1) January 1, 2021 hospital price transparency became the law of the land; 2) CMS recently (read: 3+ years later in 2024) "ramped up" enforcement of said law; 3) CMS fined this month ( see attached notice) Jackson Memorial Hospital in Miami, FL ( owned by both a not for profit hospital and not for profit educational institution whose medical school uses the hospital as a teaching facility) $877,122 ( arguably only one serious ER claim) for various hospital price transparency infractions such as '"I cannot find the list of shoppable services" and " I am having a very difficult time finding where your list of prices for your medical services is located on your website." Minor violations ( i.e. "what can you do to us anyways?") 4) The check is ordered to be made out to the US Treasury; 5) Hospital price non-transparency pays because we all pay for it anyway; 6) Although I remain convinced that ten University of Miami computer science majors, unlimited beer and pizza, $10,000 cash ( "NIL money") for each individual plus a campus wide blowout toga party could effectively get the job at hand done and assist the facility to comply with the transparency rule for much less than $877,122

Negative online reviews for Urgent Cares. Pleasing patients in the UC is tough. They come in at their worst and they interact with a lot of moving parts. It takes one wrong step to lose their faith. The negative reviews an UC accumulates also dictates the kind of talent they'll recruit. Your run-of-the-mill MA, MD, PA, or Tech will take a job based on the pay. The top-notch staff will look at your online reviews, read the online reviews, do a dry run at your facility, and then decide if they want to work for you. And the low-performing staff will recommend their friends to come work for you and the high-performer will do the same. One thing begets another.

“What did your doctor say to you?” That is usually what I tell patients in disbelief when they tell me the stories of what physicians said to them. I have YET to meet an actual patient that cannot VERBATIM tell me what a physician said to them at some point of their life and sadly, most of the time it is not uplifting but quite damaging. WORDS have power. They are not just useless things to fill the time or air space, but they are so powerful. As physicians and healers, it is both the WORDS, the tone, the mannerism, and the energy behind it that carries a lot of weight and power. We cannot take words back. Yes, we can apologize, but once the cat is out of the bag, the damage is done. This is why communication and understanding the power of communication is critical. We can all say things, facts, diagnosis, outcomes, prospects but HOW we say it as well matters. We can all say things but when we are lacking compassion and LOVE, the outcome is SO MUCH different. I am keenly and personally aware of how physicians speak to patients or to one another and I can say this: WE CAN DO BETTER! We need to do better. We truly must begin with awareness, then root with compassion and start to heal. We cannot heal others when we are not healing ourselves. And it is a journey. But we need to take the first step. It's a common sight among physicians: complaining about Medicare cuts. Why do physicians complain so much about Medicare cuts? There are a few reasons: 1. They seem to happen every year. Most people that work at a company are used to getting raises as time passes, some get them yearly. Medicare cuts are like getting a 3% decrease in your pay every year. While 3% may not seem horrible, since they seem to happen yearly, they have added up to a lot overtime. 2. Most health insurers base their payments off of what Medicare pays. For example, UnitedHealthcare may pay 150% of Medicare, so a 3% cut from Medicare turns into a 4.5% cut from UHC. And since the Medicare cuts are universal, they are applied across the board. Some might argue that physicians make too much money, and we need to stop the overspending from Medicare. Physician pay makes up about 7% of the total health spend, while administration makes up far more. That stuff gets more expensive every year. Medicare still can't negotiate most of its drug pricing... In other words you could cut physician pay down to $0 and you would still have all the major problems with overspending and waste that we have with Medicare. Solution? As physicians we are good about complaining, and very bad at taking action. I've opted out of Medicare (along with every other insurance). Perhaps if more docs did that, the story would change. Whether docs opt out or they don't we will lose them anyways to burnout as low reimbursement drives high volume practices. #Medicare #healthcare #medicine
