Congratulations to Rhafia Bucoy, DBA, MSN, RN, RN-BC, Regional Clinical Practice Consultant, Perioperative Services, NCAL Kaiser Permanente, recipient of The DAISY Foundation DAISY Nurse Leader Award.   Rhafia Bucoy is NCAL Perioperative Services’ clinical practice and financial performance consultant. She serves as an OR nurse informaticist and analyst, and she is definitely everyone’s “go-to person” for everything Perioperative data, clinical staffing, OpTime build, and all other things. Rhafia is the nurse in the background who silently gets things done, from report build to data mining, analysis, and presentations. She is the clinical liaison to the national teams for KPHC, national product council, and revenue cycle. She is responsible for the clinical testing and auditing of electronic med record documentation.   Despite the overwhelming demand for reports and other competing priorities, Rhafia manages them well and always willingly helps others. She has collaborated with revenue cycle in closing out unposted surgical logs and ensuring accurate implant documentation. She has also recently agreed to close out records related to bedside procedure recovery to ensure accurate capture of workload and patient throughout in the post operative space.   Rhafia serves as the clinical lead and resource for all of NCAL’s surgical system administrators as she lends her expert consulting hat in the OpTime domain from case creation to case closure and posting. Rhafia partnered with supply chain on the implementation of Case Pick App in 19 NCAL facilities, 11 of which went live in 2023. This application helps accurate accounting of supplies used in each surgical case leading to cost valuation and eventual supply standardization. With a few more sites going live in 2024, procedure card maintenance is key and Rhafia recognized this gap in some locations. With this information, she took the initiative to reinvigorate and develop training modules on procedure card updates. She partnered with the national training team and launched the first facilitator-led training in January of 2024.   Another important work that Rhafia is leading is the implementation of the Preop and PACU bay assignment to capture both medical and surgical workload in the perioperative spaces to inform staffing decisions. Her collaboration with the workforce and finance teams is critical in this important work.   Rhafia is a self-directed and a silent nurse leader in the Perioperative space. Her contributions to KP NCAL nursing can easily go unnoticed but definitely worthy of a recognition. Elevating Chronic Care Management through Nurse Care Managers As healthcare continues to pivot towards value-based care models, the role of #NurseCareManagers (#NCMs) in Chronic Care Management (#CCM) has never been more pivotal. NCMs are at the forefront of identifying eligible patients, developing personalized #careplans, and ensuring seamless #carecoordination. But how does this translate into better Quality Payment Program (#QPP) scores and increased incentive payments? First, by identifying patients eligible for CCM, NCMs ensure that those with multiple #chronicconditions receive continuous, comprehensive care. They not only enroll patients but also provide them with the necessary education about the benefits of such programs, leading to higher patient engagement and satisfaction. Secondly, care coordination is the heart of CCM. NCMs work tirelessly to connect the dots between various healthcare providers, ensuring that the patient's care plan is executed flawlessly. This includes managing medication, scheduling follow-ups, and monitoring the patient's health status. The seamless execution of these plans leads to improved patient outcomes and reduced hospital readmissions, which are critical metrics in determining QPP scores. Lastly, meticulous documentation and correct coding of CCM services by NCMs ensure that providers are reimbursed accurately for the care provided. This not only supports the financial health of the practice but also reflects the quality of care in QPP scores. In essence, NCMs are invaluable in navigating the complexities of chronic care, enhancing patient health outcomes, and steering healthcare practices towards higher QPP scores and increased incentives. This concerns me. I take no political sides, but nursing associations are supposed to represent all nurses. There is a reason they haven’t endorsed a candidate. It’s not good business. What about nurses who don’t agree with the admin? Or have differing political views. Do you think the initiative further divides people? Nurses are allowed to be great nurses but also be republican. Or independent or not agree. What do you all think? Should nursing association endorse candidates? If so, are they really considered organizations for all nurses now? Media and influencing are HUGE revenue areas. They might not have exchanged direct cash, but media endorsement is the same thing as a generous contribution. Want to kill the nurses power at moving bipartisan legislation? Endorse a candidate. Want to make nurses seem even less credible to people who really understand the law and how legislation works? Endorse a candidate. Our power is in our ability to focus on care and what’s ethically correct. Not by choosing a party. Everyone knows the candidates are not working alone. That’s why she’s declined solo interviews. She can’t speak openly until they tell her what she should say. Wake up America. Wake up. #healthcarelobbyists

In California, a well-intended program designed to help nurses struggling with addiction has become a trap for those it aims to assist. The system, with its rigid rules and exorbitant costs, ensnares even those without substance abuse issues (people who even evaluators say aren't alcoholics, etc), lumping them together with those needing particular kinds of support. This approach not only undermines the program's effectiveness but also perpetuates a system that punishes rather than heals, driving many to avoid the program. Physicians' lobbyists negotiated not to be included in the program, so we didn't even know how many doctors with substance abuse issues are in need of care. It's a microcosm of the macrocosm, reflecting multi-tiered structures, programs that go on and on without end, and people who've long recovered in some ways forced to pay the price by for-profit companies that have little accountability to anybody. The article highlights that doctors in California are not subject to the same stringent recovery program as nurses. Here's why: * Powerful Lobbying: The California Medical Board, which licenses doctors, actively resisted the program, deeming it too punitive. * Alternative Program in Development: They are pushing for a separate, potentially less harsh, recovery program specifically for doctors. This means that while nurses face career-ending consequences and financial ruin for substance abuse issues, doctors have a different path – one that may offer more leniency and protection. This raises several concerns: * Double Standard: It creates an unfair system where different rules apply to different groups of healthcare professionals. * Patient Safety: If doctors with substance abuse problems are not adequately monitored and supported, it could potentially jeopardize patient safety. * Equity and Fairness: All healthcare professionals should be held to the same standards of accountability when it comes to substance abuse and patient care. The article calls for a more equitable system where all healthcare professionals, including doctors, are subject to fair and effective recovery programs that prioritize both patient safety and the well-being of the professionals themselves. “We failed her miserably. She left OR nursing altogether.” The healthcare industry is facing a critical nursing shortage, and inadequate training is driving many nurses away from the profession.💔 During the pandemic Cynthia L. Garza, DNP, RN, CNOR, was the Director of Surgical and Endoscopy Services in a brand new 59-bed hospital with 3 ORs, 2 endoscopy suites, and a cath lab—and the facility lost 60% of its perioperative nurses. Great work Jana. The vacancy crisis is real. Nurses, ask yourself, what is the change to culture that is needed in retaining staff? Something that does not focus on wages. Think about how your nurses and staff feel. What do they need to do thier work and feel safe. When you find that answer then it is our duty as leaders to remove that barrier so that our nurses and staff feel safe, heard, and honored so they can perform their work. Nursing is not easy, let’s not compound it with poor culture.

I totally agree with you Tom Parker on many fronts. A one-size fits all staffing model unfairly ties a facility's proverbial hands behind their backs given the need to take into account resident's diverse requirements, geographic/demographic differences, facility size and core competencies within a building. All of that has to be dealt with before any consideration can be given to team member preferences to promote retention. Flexibility and adaptability are essential for long term care providers to survive.
