Information Systems Research (ISR) has recently published a new article to the Articles in Advance page:   Article Title "How Hospitals Differentiate Health Information Technology Portfolios for Clinical Care Efficiency: Insights from the HITECH Act"   Authors Jessica Pye (deceased), Arizona State University Arun Rai, Georgia State University John Qi Dong, National Technological University, Singapore   Problem definition We examine how hospitals differentiate their HIT portfolios in response to cost inefficiencies in clinical care processes and HIT policy uncertainty.   Relevance Adapting HIT portfolios to address cost-based shortfalls and policy uncertainty is crucial in shaping service delivery and impacting care costs, making it a key concern for policymakers and hospital decision-makers. Theoretical foundations We combine behavioral theory and institutional theory to explain how hospitals differentiate their HIT portfolios through idiosyncratic learning in response to cost-based performance shortfalls and shifting isomorphic pressures from changes in HIT policy uncertainty.   Methodology We constructed a panel dataset of 11,316 hospital-year observations from 3,319 U.S. hospitals over 2007–2014 to investigate how hospitals’ HIT search differentiation is influenced by cost-based performance shortfalls and policy uncertainty during the phases of the HITECH Act, from conceptualization to enactment to enforcement.   Results Our research provides a key insight: Moderate policy uncertainty, particularly during the HITECH Act's phases, significantly shapes hospitals' search for novel HIT solutions to address performance shortfalls. Hospitals enhance their search differentiation when uncertainty decreases from conceptualization to enactment but reduce it as uncertainty diminishes from enactment to enforcement.   Contributions Our findings surface the utility of combining the behavioral theory of the firm with institutional theory to explain the interdependence among a hospital’s cost-based performance shortfalls, HIT policy uncertainty, and HIT search differentiation. The Health Data, Technology, and Interoperability: Patient Engagement, Information Sharing, and Public Health Interoperability rule, or HTI-2, builds on the agency’s long-term work to improve interoperability and information sharing between providers, payers and the public health ecosystem, said Office of the National Coordinator for Health Information Technology (ONC) head Micky Tripathi during a press conference.   “It doesn’t make sense for us to have silos separating public health information from provider and clinical information from payer information. Follow along for the full breakdown! 📢 _______________________________________________________________ Office of the National Coordinator for Health Information Technology (ONC) said on July 10 that HTI-2 builds on the work of HTI-1 and #TEFCA to continue the march towards full interoperability. 🎙️Micky Tripathi, National Coordinator at ONC, said that HTI-2 will “expand the horizons and the capability of interoperability”. Here are some highlights of key parts of HTI-2 that do NOT deal with #InformationBlocking. 1.      Certification of Public Health IT systems- ONC will begin to certify public health-specific IT systems and products to improve the interoperability between healthcare providers that are using certified EHR products and “native” systems developed by, and for, public health agencies. ONC says a primary goal of this certification is to establish a baseline for functionality of public health IT systems.    2.      FHIR APIs- ONC states that 97% of hospitals and 80% of ambulatory providers have access to FHIR APIs but usage is frustrated by the inability to discover the APIs and to automatically connect to them. ONC intends to require certified EHRs to support dynamic client registration as used in TEFCA. 🎙️Steve Gravely says, "eHealth Exchange expressed significant concerns with the TEFCA requirement of the UDAP dynamic registration standard, so we should take a close look at the proposals in HTI-2 and consider submitting comments on them." 3.      USCDI v.4 will be required for certification effective 1-1-2028 and v.3 will be retired.

Today the Centers for Medicare & Medicaid Services (CMS) published the request for applications (RFA) for the ACO Primary Care Flex (PC Flex) Model, which will test #primarycare hybrid payments within MSSP beginning January 1, 2025. To apply, ACOs must submit an MSSP application (as an initial, re-entering, or renewing/early renewing applicant) and a supplemental PC Flex application questionnaire. The MSSP application is open now through June 17 at 12:00pm (noon) ET. The PC Flex supplemental application must be submitted by August 1, 2024. CMS will host a model applications webinar on June 6 at 2-3:30pm ET. Key model details: > The model will run January 1, 2025 – December 31, 2029. > Eligible participants are low revenue ACOs using prospective assignment that begin a new MSSP agreement starting Jan. 1, 2025; ACOs cannot simultaneously participate in Advance Investment Payments (AIP). > Participation is at the ACO level and all ACO participants and their primary care providers will be required to participate; fee reductions will be applied at the TIN/NPI level. > In addition to the MSSP application and PC Flex questionnaire, applicants must submit a spend plan, roster of non-physician practitioner data, and assurance of ability to repay; applicants may withdraw the PC Flex application at any time prior to signing the Participation Agreement (December 6 – 12, 2024), and may withdraw the PC Flex application without impacting the MSSP application. > Participants will be required to submit a health equity plan, collect and report health equity data, and secure a repayment mechanism (separate from MSSP repayment mechanism for 2-sided tracks). > ACOs will distribute the Prospective Primary Care Payment (PPCP) to their participant primary care providers, FQHCs and RHCs.
