Supported the development and dissemination of health information technology standards amongst US organizations and more specifically amongst African nations and organizations looking at developing national eHealth Strategies. While the primary focus was HL7 standards for transfer of clinical and administrative data between software applications used by various healthcare providers, we also advocated the value of DICOM (standards for storing and transmitting medical images enabling the integration of medical imaging devices such as scanners, servers, workstations, printers, network hardware, and picture archiving and communication systems (PACS) from multiple manufacturers ) and SNOMED CT (standards for clinical terminology that is used by physicians and other healthcare providers for the electronic exchange of clinical health information) in conjunction with HL7 standards.

The research problem addressed in this manuscript was premised on the impact of going beyond implementation to the activities that define adoption of eHealth and how that could potentially accelerate the attainment of United Nations Sustainable Development Goal 3 for health. The research was conducted from the perspective of healthcare professionals in Kenya working as general practitioners. The purpose of the study was to collect and analyze data that would inform and catalyze the shift from a focus on implementation to adoption of eHealth solutions by policy makers, foundations, donor agencies and stakeholder vested in strengthening Kenyan and similar LMIC health systems through health information technology. Guided by the Technology Adoption Model, the Carayon Systems Engineering Initiative for Patient Safety, Leadership Theory, eHealth strategy and a clear distinction between adoption and implementation, the researcher examined the experiences of EMR/EHR users in Kenya. Based on a qualitative case study design the findings demonstrated that for adoption to become the norm, barriers need to be overcome including additional EMR functionality, health informatics education, and more strategic orientation from the Ministry of Health leaders in developing an enabling environment. Beneficiaries of this research include Kenya’s and other LMIC Ministries of Health (MOH), stakeholders such as the Palladium Group, the Bill and Melinda Gates Foundation, the Rockefeller Foundation, Partners in Health (PIH), the United States Agency for International Development (USAID), and the World Health Organization (WHO). The results, based on medical practitioner feedback, provided the latter organizations with critical information to maximize the return on their investments in eHealth solutions for Kenya and other LMICs and meet SDG3 goals.

The nascent nature of blockchain and the popularity of cryptocurrencies (cryptos) make it easy to overstate the benefits of the underlying technology and overlook the associated technology adoption theories. The architectural redesign in how value is created and secured in various markets has on the most part been ignored; regulatory involvement has been ignored and resistance from current middle-man institutions concerned about losing their revenue streams has not been adequately accounted…
The nascent nature of blockchain and the popularity of cryptocurrencies (cryptos) make it easy to overstate the benefits of the underlying technology and overlook the associated technology adoption theories. The architectural redesign in how value is created and secured in various markets has on the most part been ignored; regulatory involvement has been ignored and resistance from current middle-man institutions concerned about losing their revenue streams has not been adequately accounted for. One must understand that the reality lies somewhere between the utopians and the detractors of BKN and therefore make a concerted effort to get beyond the exaggerated blockchain hype and focus in on the assumptions challenged by BKN technology. In essence that means understanding the old adage in business; if it saves money, makes money or gives a significant competitive advantage its worth investing in. For blockchain technology and irrespective of what level or maturity an organization has achieved a fundamental question is what costs does blockchain eliminate or substantively reduce? The response to this is a clear means to identifying the opportunities associated with blockchain. Those organizations or applications that leverage this benefit are likely to succeed just as those that claim unrealistic benefits from BKN technology, are likely to fail.

The opportunity for the application of BKN technology in low and middle-income countries is tremendous; however, a key difference is that while the commercial focus is on outstanding technology challenges, the LMICs are focused on the trust aspect of block chain. With respect to the 2030 development agenda, BKN is a significant breakthrough that will make a huge impact on and add value to any transaction that requires trust. This is very likely to support the Sustainable Development Goal (SDG) agenda adopted at the beginning of 2016 to run through 2030, designed to end poverty, improve health significantly and ensure prosperity for all. For SDGs there is substantive potential for BKN in areas such as land registry (given that according to the World Bank 70% of the global population lacks proper land titling or demarcation), health, digital identity, and finance for small and medium enterprises (SMEs) (Underwood, 2016).

Business Intelligence must, however, be coupled with critical healthcare intelligence that is relevant to the respective rural environment. Current measures with their corresponding exclusions may now suffice. However, we must begin to look at the demands or rural health and focus on relevant, future intelligence measures for reporting by CAHs or rural healthcare facilities. The absence of relevant intelligence can no longer suffice for CAHs /rural hospitals failing to meet certain quality outcomes. By encouraging adoption of future rural intelligence measures and public reporting of the same, CAHs /rural healthcare service organizations (HSOs) will be guided to safer, more effective, patient-centered, , and efficient care.
