Due to advancements in medical and surgical care, the survival of patients with congenital conditions into adulthood has dramatically increased. However, as these individuals transition to adulthood, their unique physiology, chronic complications, and evolving care needs create significant challenges for their management when they are admitted to adult intensive care units (ICUs). Welcome to the newest members of our Sound Long-Term Care Management (SLTCM) clinical team — Nikki Powell, RN, Chelsea Wang, BSN, RN, and Lori Weatherby, BSN, RN, SMQT.     We’re thrilled to have you aboard as our Quality Improvement Nursing team, working closely with our Chief Medical Officer Thomas Kim, MD, MMM, FHM and Associate Chief Medical Officer Sandeep Yadavalli, MD, FACP, FHM to help bring better care and outcomes to long-term care and assisted living residents.    SLTCM has a full clinical team that understands the complexities of caring for this vulnerable population.
