President & CEO Medical Home Network ⟶ Building Healthier Communities By Transforming Care and Enabling Success Under Value Based Care
I’m back from ViVE and grateful for the opportunity to meet like-minded colleagues who are passionate about making value-based care work for providers, payers and patients. I enjoyed joining Erin Richter Weber, Chief Policy & Research Officer, CAQH Christopher "Chris" Caramanico, CEO, Elligint Health and Brent Asplin, MD, MPH, CEO, Gather Health, on our panel, Value-Based Care Insights: VBC Models: Let's Talk Success! The future of value-based care is not just about improving quality and reducing costs—it’s about fundamentally redesigning how care is delivered. Success will depend on: - Flexible payment models that incentivize proactive, whole-person care rather than episodic treatment - Care team redesign that maximizes provider capacity and ensures every team member works at the top of their license - Digital innovation that automates administrative tasks, enhances coordination, and improves access to care - Scalability and policy support that enable APMs to drive long-term sustainability and health equity Excited for the road ahead and inspired by the collective commitment to making value-based care work. Let’s keep pushing for meaningful change—together! #ViVE2025 #ValueBasedCare

President & CEO Medical Home Network ⟶ Building Healthier Communities By Transforming Care and Enabling Success Under Value Based Care
We recently hosted our third MHN Talks in Ten webinar with Ryan H., principal at Leavitt Partners who also helps lead the CARIN Alliance. Ryan shared illuminating insights on the promise of digital health, which is the use of technology to improve care delivery, outcomes and efficiency. Ryan discussed the modernization of data exchange, which is key to improving FQHC workflows, reducing administrative burden, and enabling true interoperability across healthcare systems. He spoke passionately about why it’s important to develop an API strategy and leverage Fast Healthcare Interoperability Resources (FHIR)—an interoperability standard developed by HL7 to facilitate the electronic exchange of healthcare data. My three key takeaways from the session: 1. Data exchange between providers, payers, and patients is now mandatory, not optional. This provides both an opportunity and a challenge for FQHCs, so staying up to date on policy changes and application programming interfaces (APIs) is essential to conducting our work. 2. Modern APIs using the FHIR standard provide better structured data for FQHCs for value-based care use cases, quality reporting, analytics, care delivery, innovation, and secure data exchange. Thoughtful implementation of APIs can help lower administrative burden for providers and patients. 3. The CMS 0057 Interoperability and Prior Authorization Rule mandates that anyone who bills CMS send necessary clinical and claims data to each other (payers, providers, and patients) via APIs by 2027. A positive outcome of this Rule is that prior authorizations will be processed almost instantaneously instead of over weeks. A big thanks to Ryan for inspiring us on MHN Talks in Ten! If you missed the talk, you can watch it here: https://lnkd.in/gniC2dqh Stay tuned for our next MHN Talks in Ten — which, as always, we promise to be a short talk with a big impact! #MedicalHomeNetwork #MHNTalksInTen #ValuebasedCare #FQHC
