Our Mission The mission of the Care Transformation Collaborative is to lead the transformation of primary care in Rhode Island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve. To engage providers, payers, patients, purchases and policy makers to develop high quality, family and patient centered medical homes providing health care in an affordable, integrated healthcare system that promotes active participation, wellness and delivers high quality comprehensive health care dedicated to data-driven system reform. Our Vision Rhode Islanders enjoy excellent health and quality of life. They are active participants in an affordable, integrated healthcare system that promotes wellness and delivers high quality comprehensive primary care. The Care Transformation Collaborative of Rhode Island has a growing impact across the state, and includes: • 124 primary practices, including internal medicine, family medicine, pediatric practices and 10 primary care practices are part of integrated behavioral health initiative. • Approximately 695,000 Rhode Islanders receive their care from one of our practices. • 800 providers across our adult and pediatric practices. • Investment from every health insurance plan in Rhode Island, including private and public plans. • All Federally Qualified Health Centers in Rhode Island participate in our Collaborative. • CTC practices demonstrate lower total cost of care by $217 million in 2016 when compared to non-patient centered medical homes in Rhode Island, according to data from the state’s All-Payer Claims Database.

✨ Welcome Mitchell Irving, MPH, to the CTC-RI Team! We’re excited to introduce Mitchell Irving, MPH (he/him) as CTC-RI’s newest Program Coordinator! Mitchell will play a key role in advancing Rhode Island’s primary care workforce, supporting initiatives to expand clinical training site capacity, enhance curriculum development, and engage providers. He’ll also be working with several CTC-RI committees—including the Clinical Strategy Committee and Breakfast of Champions—and helping to launch a new training series on pediatric anxiety. Mitchell brings a strong background in public health, research, and healthcare administration, with experience across academic, healthcare, and community settings in the US and Canada. He holds a Bachelor’s in Anthropology from Central Connecticut State University and an MPH from the University of Connecticut, where he focused on community-based research and international health. His past work has spanned physician well-being, virtual care evaluation, HIV stigma research, and the history of healthcare institutions. 🗣️ In his own words: "I was thrilled to find such a unique opportunity at CTC-RI that blends my academic background in public health and my varied professional experiences in mixed-methods research, healthcare administration, and clinician well-being. Over just my first few weeks here, it has been so exciting to learn about the range of projects and substantive impact that CTC-RI’s work has had in RI. In February 2024, Governor Dan McKee signed Executive Order 24-04 establishing the State Health Care System Planning (HCSP) Cabinet to develop a high-quality, affordable, equitable, accessible, culturally, and linguistically appropriate health care system capable of meeting the varied needs of all Rhode Islanders. CTC-RI Executive Director, Debra Hurwitz, is on the Advisory Council for this Cabinet. Recently, EOHHS released their Health Care System Planning 2024 Foundational Report. It compiles input from a vibrant stakeholder process, including the participation of representatives from 11 state agencies and over 40 provider organizations, businesses, labor organizations, current health care planning tables, and community-based groups. It provides baseline data on the Rhode Island health care system, including a summary of community need and leading population health indicators; lays out initial information on the sectors and cross-cutting issues at the heart of the proposed Health Care System Plan; and provides recommendations created by the vibrant public/private process that EOHHS began in March of 2024. CTC-RI has been working with TEAM UP Scaling & Sustainability Center and the Hassenfeld Child Health Innovation Institute - Brown University on expanding integrated behavioral health in pediatrics by leveraging Community Health Workers (CHWs) as part of the care team. With funding support by UnitedHealthcare, Blue Cross & Blue Shield of Rhode Island, CTC-RI has been able to fund 6 practices to participate in this initiative.
