The highly anticipated Darzi Report was published yesterday. Commissioned by the incoming Secretary of State for Health and Social Care, this Independent Investigation of the National Health Service in England hoped to understand the healthcare system's performance and challenges. There will undoubtedly be many detailed analyses of the report in the coming days. I have been keen to reflect on how other health systems with similar delivery values as the NHS, particularly low/middle-income countries, can leverage these learnings and recommendations and perhaps anticipate some of these challenges in advance. My three takeaways with introspection are: Firstly, appreciating the demographic shift to an ageing society and the burden of non-communicable diseases will exert enormous pressure if huge primary, preventive care and social care investments are made from the offset. (the best time is now) This must be coupled with a whole-of-government approach addressing broader inequalities and social/commercial determinants of health. Secondly, while leveraging digital transformation and artificial intelligence may have enormous transformational potential to improve innovation, service productivity and delivery efficiency, the eco-system must be conducive, empowered and responsive to address inevitable challenges while governed by a robust framework and considered policies. Finally, having patients and healthcare providers at the front and centre of every conversation about health service improvement and delivery needs. Fundamentally, a responsive health system re-engages, acknowledges, empowers, and drives change guided by these voices. Link to the report below. GMC annual training survey Pulse has published a piece reporting on the GMC’s annual training survey, published today. The article states that ‘plans for expanding the medical workforce will fail if the NHS does not prioritise trainer capacity and support, the GMC has warned’. The article focuses on GPs but also states: ‘According to the GMC, these figures show that there are ‘clear signs of strain’ among the medical trainer workforce, which will only intensify as training places increase’. It will need a change in mindset and an adoption of XR and AI approaches to learning. Three quarters (75%) of the public support sharing some of their personal health data for the development of AI systems in the NHS, according to a survey commissioned by think tank the Health Foundation. Interesting findings. Anything you do these days, you get asked for permission to share the data. This helps organisations understand their customers, and the healthcare sector is no exception. Although some individuals remain sceptical about data privacy, I believe we live in an era when data sharing is becoming more open. DPN Research discovered in 2022 that the younger generation is more likely to feel comfortable sharing their data, which actually makes real sense when you think about it. Back to the NHS, results show that 59% were willing to share data on areas such as their eye health, 58% on medicines they are taking, and 57% on long-term illnesses they live with. However, the percentage of phone-tracked data is lower, reaching 47%. People named the ability to see and talk to NHS staff as their most important consideration about technology use in healthcare, with 39% people aged 65 years and older naming this as their most important consideration. The poll found that around two-thirds of the public have either high or moderate levels of trust in the NHS with their health data. #digitalhealth #artificialintelligence #healthcareinnovation #healthcaretechnology

Many people f**k up freedom of information requests and p**s off the NHS. Around ten years ago I watched and wrote some helpful guidance for NHS commissioners when dealing with FOIs from pharma. The CCG teams were not happy. They were overwhelmed with requests on all kinds of things, and it was usually bloody obvious who was making the request. BUT The commissioners were public, tax-payer funded bodies who often refused to take meetings, and were not inclined to share information. So they created the conditions that kind of forced the companies to do it... ... thus pissing them off. FOI requests are very useful, but DON'T BE A ROOKIE! This Wednesday I'm running a session that will cover how and where to get intel from the NHS, that helps you to engage with the NHS in the right kind of way. The 90m session will be covering.... Our latest study has implications for all nurses who utilise algorithmic tools, such as early warning systems, in their practice. Although experienced nurses appreciate the support of early warning tools, most value patient safety above the tools and rely on their higher-order thinking. In the UK's "National State of Patient Safety 2024" report, significant issues have emerged regarding patient safety, notably in #maternalcare and #womenhealth. The report highlights an alarming rise in maternal and neonatal mortality rates, with stark inequalities, particularly affecting black women. The primary cause identified is the delay in accessing #urgentcare. Even high-income countries are seeing an increase in deaths due to lack of #healthcareaccess. The solution is not solely about increased funding. It's about creating ecosystems that foster health genesis, learning, and sharing experiences. In response to this urgent need, #communitiesofpractice are being developed to accelerate the adoption of proven strategies and establish structural mechanisms to support them. The answer lies in sharing, co-producing, and #co-designing. #Thinkingdifferently Sometimes, it is hard to know where to start with the many challenges faced by the #NHS. The challenges are global, and the scale of risks for the population is not easily solved by more money and more staff. It is easy to focus on the problems, the diagnosis, and the analytics. It is easy to run to the tried and failing cost improvement programmes (CIPs). It is easy to fall into the tsunami of healthcare tech hoping for magical solutions to the workforce and productivity issues. It's clear that our current thinking, mental models, and approaches are not sufficient to address the challenges we face. We need to be open to new thinking to truly tackle these issues. Had the privilege of participating in a Structured Judgment Mortality Review (SJR) at my Trust Bolton NHS Foundation Trust. This insightful process involved both qualitative and quantitative reviews of patient care during their last admission. The retrospective nature of the review allowed me to reflect deeply on my own practices and identify areas for improvement. By learning from these cases, we can spot trends in patient care, enhancing our management plans and ultimately improving future outcomes. While we can't change the past, this valuable learning experience empowers us to elevate the standard of care for our patients moving forward. #NHS #Healthcare #PatientCare #ContinuousImprovement #LearningFromExperience

How do electronic prescriptions contribute to the reduction of medication errors? Elliott et al.'s (2023) study, "Estimating the impact of enabling NHS information systems to share patients' medicines information digitally," investigates the potential benefits of integrating interoperable prescription information systems within the NHS. Key findings suggest that implementing a digital, interoperable system could reduce transition medication errors by up to 40%, leading to fewer adverse drug events, reduced hospital admissions, and cost savings for the NHS. Although digital integration holds the promise of long-term benefits, it is important to consider the substantial short-term disruption and costs that it can bring. Additionally, the effectiveness of these systems relies heavily on consistent and accurate data entry, which is still subject to human error (Akinola & Telukdarie, 2023).
