    - Included medical, nursing, pharmaceutical, rehabilitation and therapies (including physiotherapy and occupational therapy) and social services staff on ward rounds
    - Introduced care pathways for patients with head and spinal injuries or needing cardiac intervention.
    - Dispensed drugs direct to patients on the ward to reduce the time patients wait for their medication.…

    - Implemented discharge decisions following senior multi-disciplinary ward round assessment of the patient
    - Included medical, nursing, pharmaceutical, rehabilitation and therapies (including physiotherapy and occupational therapy) and social services staff on ward rounds
    - Introduced care pathways for patients with head and spinal injuries or needing cardiac intervention.
    - Dispensed drugs direct to patients on the ward to reduce the time patients wait for their medication. Pharmacists also benefit from a better knowledge of patient’s drug histories and we have reduced inappropriate dispensing of medications.
    - Identification of discharge date and full plan of care no more than 12 hours after admission.
    - Informed patients and carers about the expected date and time of discharge and involved them in the process early in their stay
    - Planned diagnostic tests and other interventions to avoid delays in treatment with local standards for response times for referrals to key diagnostics, including a same-day service for imaging.
    - Reviewed patients at least twice-daily and any changes to the expected date of discharge is documented
    - Introduced rapid access outpatient follow-up clinic
    - Measured the effectiveness of the discharge process

    each story are published standards, metrics and guidelines from Royal Colleges, NICE,
    London Health Programmes and others that have extensively reviewed clinical research and
    worked with patients and clinicians to describe what good quality of care looks like.
