Matthew Custance The current system for deciding which/ when/ where to build new hospitals in the UK clearly isn’t working, and needs to change. PFI in my view didn’t work either, as it bundled building, FM and other services into complex contracts and the PFI consortia quickly became good at stealing sweeties from tge kiddies letting contracts. Has anyone done a comparison of the end to end process for planning and building a new public hospital in UK vs a range of other developed countries with either social health insurance or Bevan systems? My hypothesis is that in the UK there is a much greater degree of political and treasury involvement, leading to delays, decisions which are sub-optimal at a local level, and excess time and money spent planning projects.

The report rightly highlights the need for a focus on clearly defined national health innovation priorities, incentivising the partnership working required to deliver innovation, and improved alignment and shared accountability for innovation adoption performance. Todays report from the Innovation Ecosystem Programme (IEP) outlines ways the NHS can further develop and implement innovations in healthcare. There is an opportunity to embed the report’s recommendations into the forthcoming 10-year health plan, they include: Building skills, capabilities and culture and unlocking time the help the NHS workforce prepare for future ways of working. Developing commercial approaches to share value and adoption in testing innovation. Building robust evaluations to assess the impact of innovations. We welcome the report and look forward to continued collaboration with colleagues across the East of England

It’s not the delivery of pathways that will “fix the NHS” but an appropriate understanding of the fundemnetal problems, Capital versus Revenue spending, investment in workforce development, single national procurement approach, national digital leveling up. Great article Nick Triggle, just the tip of the iceberg. There could be 600 approaches to fixing the NHS but not without any long term approach to development of a digital workforce in both delivery of digital enablement & enduser training. We need gaurentees of recurrent revenue to support both workforce & for IT infrastructure development. NHS Trusts hands will remain tied and living within a pandemic of short-termism if we don’t move away from Capital funding mechanisms. The entirety of care pathways rely on multiple providers across primary, secondary & social care many of which have subsequent private contracts. Legacy commissioning contracts need to be unpacked & redesigned with the patient put at the heart of it before we dive into reimagine new ways of “fixing” the NHS. This myriad of commissioned contracts written over the past 20 years to ensure services remained upright. Trusts having the expertise & time (again revenue please) to reimagine any healthcare delivery takes long term planning.

In the UK's "National State of Patient Safety 2024" report, significant issues have emerged regarding patient safety, notably in #maternalcare and #womenhealth. The report highlights an alarming rise in maternal and neonatal mortality rates, with stark inequalities, particularly affecting black women. The primary cause identified is the delay in accessing #urgentcare. Even high-income countries are seeing an increase in deaths due to lack of #healthcareaccess. The solution is not solely about increased funding. It's about creating ecosystems that foster health genesis, learning, and sharing experiences. In response to this urgent need, #communitiesofpractice are being developed to accelerate the adoption of proven strategies and establish structural mechanisms to support them. The answer lies in sharing, co-producing, and #co-designing.
