Interesting report by the Tony Blair Institute: Person-centred care data strategy The success of this initiative hinges on the collective effort of policymakers, healthcare providers, and technology vendors to embrace and implement this approach. Key to this will be adoption of the correct standards (including FHIR, openEHR, IHE and SNOMED), together with a curated set of national information models. I think this is the time to make this a global strategy, with the same foundation build on the same standards and global information models, open-source and privacy first. #health #zorg #data #iza

As a baseline NHS & social care tech must be functional and usable- reliably do what clinicians & patients need in a way that is intuitive. Fail with this and no matter how exciting the tech the effects will be muted at best & more likely detrimental.

Integrated care systems are showing big variations in their ability to make #effective use of #patient #data, according to a new report - Understanding Patient Data, in collaboration with its hosts at the NHS Confederation and its ICS Network, has published a new report on the use of data within #ICSs. It found that while some ICSs, or parts of them, have large analytical datasets containing information from a range of sources – including primary, acute, community and mental health NHS providers and local authority social care – other datasets are significantly less advanced. This creates the risk of unwarranted variations in access to data driven models of care for patients. More...

What is the variation in digital capabilities in NHS hospitals? We can see there’s a large spread this makes spreading tech a challenge & leads to variation for patients. This very sad BBC report with comments from well-respected digital health experts like Professor Joe McDonald shows it's time to get serious about Digital Health. Almost all hospitals now use electronic health records and patient administration systems. Many of these systems are poorly designed and out-of-date. This presents a huge challenge for both patient safety and clinical burnout. If we carry on business as usual the costs involved in updating these systems will be enormous. Healthcare organisations have already spent billions of pounds and dollars on the current generation of EHR systems. So how can we design and implement simpler, safer, and lower-cost systems at scale? -> Mandate modern open source technologies and open standards like HL7 FHIR -> Mandate user interface guidelines for safety critical functions (such as e-prescribing) that have scientific evidence of safety and effectiveness -> Enforce use of existing regulations for clinical safety and patient privacy -> Invest in a well-trained and knowledgable digital health workforce that works alongside clinicians to design, implement and support the safe and effective use of systems

All digital health roads lead to Rome this week! What you’ll soon be able to see on HIMSS TV: Communication is key in digital transformation, said Fay Stevenson from Barts Health NHS Trust. We had many discussions about digital maturity and the practicality of HIMSS maturity models. Liverpool University Hospitals NHS Foundation Trust achieved Stage 7, and as Graham Holland said, achieving digital maturity with EMRAM is all about improving patient safety. Cybersecurity has gotten to the next level with generative AI, it’s not just knowing about phishing anymore but also deepfakes, third party vendor safety and more, mentioned David Wall. Further more, Connected devices are the next frontier in need of protection, mentioned Tapan Mehta, Palo Alto Networks. Italy has 20 regions, hospitals are rocking with EMRAM 6, the big challenge now is that European resilience funds are giving digital transformation a boost, the challenge will come after 2025, when all the tech will need to be maintained and will require sustainable financing models, mentioned Marco Foracchia. Lisa Hanselmann presented Project Flute which aims to improve predictions of aggressive prostate cancer with a federated data appoch and by offering AI support to physician, while minimizing unnecessary biopsies, ultimately benefiting patients and reducing associated costs. Patient Information Forum (PIF) has published an update to its quick guide to using #plainlanguage in health information. The guide includes data on literacy challenges, top tips for writing in plain language and advice on use of word economy and active language. It also offers advice on communicating risks and benefits and the use of artificial intelligence (AI) in the production of health information. Digital Health and Care Wales has issued a contract notice for a Network as a Service (NasS) solution worth in excess of £2 million, designed to enable the department to “strategically interconnect its physical datacenter estate, public cloud platforms, and consumer base using a resilient underlay/transit network”. According to the notice, the NaaS solution will need to “interoperate/interconnect with the CORE solution”, helping to ensure that traffic from on-premises or public cloud platforms services is “routed efficiently”, as part of the managed network as a service offering. "Digital Health and Care Wales publishes organisational strategy for 2024-2030, highlighting need for “a digital and data revolution” “NHS in Wales is at a turning point. The challenges we collectively face are complex and ever-changing; improving health and reducing inequalities against a backdrop of the ongoing impacts of the COVID-19 pandemic and limited funding. But the opportunity to transform health and care is also significant.” The strategy sets out five missions, including ✅ providing a platform for enabling digital transformation ✅ delivering high quality digital products and services ✅ expanding the digital health and care record ✅ driving better values and outcomes through innovation ✅ being a trusted strategic partner and a high quality, inclusive and ambitious organisation. On providing a platform for enabling digital transformation, DHCW’s future model is a single data platform and a single digital health and care record, which it states will become “the foundation for all digital services and applications in Wales”. Open architecture to support innovation and the development of data standards for health and care are also highlighted as key features. Plans to 2030 include moving all data stores and services to the NDR platform to create a single national Clinical Data Repository, extending data standards and components to social care and other partners, and establishing an all-Wales framework for sharing health and social care data. Nice to see the homeland trying to move things forward (I'm half Welsh!) There's that slippery phrase "enabling digital transformation".... will be interested to see how this manifests. #diitalhealth #strategy

More important than ever that we have the Chief Data and Analytical Officers Network to sift and provide a clear narrative as we see the role out of the 10 year planned move from analogue to digital.

In 2024, there’s still poor digital maturity in a lot of NHS trusts. Printing consent forms, scanning documents onto a system and paper prescriptions that need to be run down to the pharmacy before it closes at 6 pm, are some examples of how paper is still dominant in many NHS trusts. ‘The NHS is broken’ is broadcast on repeat. An interesting panel discussion on #NHSAIStrategy held by the The Health Foundation outlined the strategic direction, development and implementation to lift the burden on the NHS The 6 main priorities the NHS will focus on are: 🧑‍🧑‍🧒‍🧒 The use of AI should be shaped by the public, patients and healthcare staff to ensure it works for them 🔬 The use of AI in the NHS must be underpinned by high-quality testing and evaluation 🌐 The NHS must focus AI development and deployment in the right areas ⚠️ The NHS needs a clear and consistent regulatory regime for AI 📈 The NHS needs data and digital infrastructure that will enable it to capitalise on the potential of AI 👩🏻‍💻The healthcare workforce must have the right skills and capabilities to capitalise on AI Health inequality was raised several times in the panel discussion. Whilst AI is already changing lives every day it is doing so in a fragmented manner. That’s why I am building the solution, in everyone’s pockets. Watch this space to see what we are developing!

IT #system #failures have caused repeated problems, and even been linked to a handful of patient deaths, at #NHS hospitals in England, according to a report by BBC News - an #FoI identified a series of problems with #EPRs. The worst being that three deaths in two trusts had been related to EPR problems. Others were that 89 trusts had logged instances when patients could be harmed due to problems with EPRs, almost half recorded instances of potential patient harm linked to systems, nearly 60 reported IT problems that could affect patient care and there were 126 instances across 31 trusts of serious harm linked to IT issues. This recent report from The Health Foundation highlights the importance of staff involvement in the development and deployment of technology within the NHS. According to the report, technology adoption is more likely to be successful if staff have greater involvement in demand signalling. This thought-provoking report provides some great takeaways, and I'm interested to hear how my network envisions achieving staff involvement in technology development and deployment within the NHS. Let's work together to ensure the success of technology adoption in healthcare. In my view implementations are only successful if there is user centred design and that the tech solves real problems faced by patients, carers and clinicians and staff.

#Ai realities and froth in #healthcare One of the The Labour Party ‘reform’ principles for the #NHS is analogue to digital. For digital one might hope it’s about the basic infrastructure and enabling rather than adding more work. It is definitely about the Ai elixir. Is the elixir real or just more snake oil? It's not been an easy few years for digital health innovators to grow their businesses, or for NHS organisations to commission them, and it might get worse before it gets better. There are more vendors chasing contracts than ever, more noise in the system, and nowhere near enough revenue available in the UK for our best startups and scale-ups to thrive. There are also cuts in funding when there should be increases e.g. the NHS AI Lab (see below) despite the pressing need for more evaluation, scale and adoption of AI into our systems. A few firms will win disproportionately more of the support than they deserve leaving relatively less for others. Accelerators will select more mature firms as they're easier to support (and more likely to win contracts). VCs will increasingly reject ventures for being too focused on the NHS. Some firms will fall into distress, if they aren't already. Many will layoff more of their people in an effort to tighten belts again without wishing to entertain a down round. A small percentage of firms will sheepishly announce their acquisitions, and mask how close they were to closing before being bought for a fraction of their previous valuations. Even big agencies are feeling the pinch. Good salespeople and experienced bid teams aren't going to be enough. This will turn firms and investors away from those targeting the NHS/UK market, and even our innovators will increasingly view foreign markets as desirable. My warning comes with some predictions; namely that we will see more startups willing to innovate on their business models to win traction (e.g. risk sharing, volume-based pricing, offering equity to NHS Trusts etc.), and there will inevitably be more innovators applying serious effort in finding new markets abroad. Elite NHS centres (which have always been more capable of revenue generation to withstand financial pressures) will collaborate more with industry and push further into international markets. Therefore the next few years will see 3 skillsets stand out: 1) Those who can support startups to scale reliably (including market access, and support to raise pre-seed/seed investment) 2) Those who can guide scale-ups to expand internationally (particularly into the USA and Saudi Arabia) 3) Those who can help health systems to commercialise their capabilities and assets appropriately for patient and system benefit, especially around real world data (RWD), public-private partnerships, and international collaboration. There really is a wealth of talent in the country, but really not that many people who excel at any one of the above, vanishingly few who can genuinely offer 2, and if you can find someone who has all 3 skillsets, then hold onto them tightly- they're special. Planned Digital Health Records in the NHS. This is what we are served up by NHS Digital…https://lnkd.in/evkz626P What is not explained here is that the NHS has outsourced this work for the so called ‘Federated Data Platform’ to US defence contractor Palantir. Yet another ’white elephant’ in the making. Yet another reason for Rachel Reeves to fritter away our taxes on a highly questionable endeavour. Think about it. For those of us working at the front line of digital health it really is akin to ‘muck and bullets’. Clinicians are having to work with badly outdated technology and much of it predicated on documents not data. Ask any frontline clinician and the story is the same: age old, clunky technologies, creating lakes of datasets, partially duplicating other datasets, so no one really knows the ‘truth’ anymore. One only has to look at the number of failures of Electronic Patient Record implementations to know what a mess will be created. I believe that something like 40% of all NHS hospitals do not have access to a working (and safe) EPR anyway! To make it worse NHS Digital has allowed a whole ‘cottage industry’ of apps for this and apps for that, further creating more ‘puddles’ of data. No common standards terminology means that nothing is understood at a system level, only within the narrow use case that the app was developed for. So this marvellous federated system is going to join all this together so we can have a total joined up digital health experience!! I’m personally disgusted that NHS Digital has been allowed to progress this ‘under the radar’ with little public debate as to whether it’s good for the UK. When the burden of the funding for this will come from the taxes off all of us in the private sector it’s even more repugnant. The investment in digital health should be for frontline healthcare with digital technologies that are truly interoperable. My business has established an exemplar of the ‘art of the possible’ in this respect. This called TCC-CASEMIX. It enables data to be created once and then re-used multiple times. It enables automated documents to be created ‘on the fly’ but always using the ‘one instance of the truth’. The automation delivers significant productivity gains. Only once have the feeder systems of health data in place (like our exemplar) can we hope to ‘join the dots’ of patient data between diagnostics, interventions and health outcomes. This investment in a federated system is a massive strategic mistake. NHS England has failed to learn the lessons of the last Labour government’s failure with the SPINE system. This will fail for exactly the same reasons. This is truly an example of ‘learn, forget, relearn’…at the private sector’s expense.
