Today, Understanding Patient Data has published a new report by White Tail Consulting based on in-depth interviews with leaders from Integrated Care Systems (ICSs) across England about how ICSs use patient data. The findings of the report show considerable variation in maturity of ICS data infrastructure, sharing and use, with some systems and areas better able to realise the opportunities of using data to improve population health. Despite challenges relating to variation across the country, capacity, organisational culture and development of data skills, and recruitment and retention of staff, participants report a desire to collaborate to drive improvements in the use of data nationally, with many examples of best practice already present. The scope for a shared care record is as simple or as ambitious as you want to make it. At the very least we could apply this here in Aotearoa to integrate primary / secondary care views of patient information. Last week at the TUANZ conference in the DHA workstream I heard yet again how frustrated primary care providers (GPs in this instance) are with not being able to view their patients secondary care record. It's as easy as onboarding a user can be (roles exist already for this use-case) leaving the complexity of active use (doing, not just viewing) to evolve as models of care are developed to support the shift-left strategy over time.

At the Care Quality Commission board meeting on 25 September, James Bullion trailed significant changes to CQC’s operations, particularly regarding its new regulatory platform: 1. Assessments: Moving away from the new platform to speed up processes and increase assessments. "...we're going to need to change the way we're interacting with our new computer system, the regulatory platform, and take an approach which for assessments works away from that platform..." ➡️ Translation: CQC is reverting to old methods (clipboards, Word, Excel) hoping not to lose personal data. 2. Registration: "...going back to our previous method of registration with people, but not losing anybody's work..." ➡️ Translation: Reverting to email-based registration to avoid losing attachments and key information. 3. Single Assessment Framework Review: Collaborating with National Care Forum for input to Prof. Sir Mike Richards' review of the Single Assessment Framework. “…you mentioned Mike Richards review of the staff and the work that Vic Rayner, Professor, Vic Rayner, is undertaking as well for the social care sector.” 4. Scoring Changes: "...not scoring at our evidence category level, but scoring at our quality statement level, for example, and not taking into account legacy scores..." ➡️ Translation: Abandoning complex evidence category scoring and instead scoring at quality statement level. Senior team couldn’t explain it to Penny Dash & inspectors asked providers to help them figure it out. Basis for new scoring unclear. 5. Regulatory Platform: "...we are still improving it. We are investing in it now, not as it were, making a final decision about it, but we do need to independently evaluate how that system performs..." ➡️ Translation: Continuing to pour money into the failed digital portal despite current non-use and hoping for the best. 6. Staffing: "...strengthening the numbers of staff in our operations service and accepting and working on the very important recommendations from the Penny Dash view about the role of the chief inspectors..." ➡️ Translation: Addressing 10% vacancy rate, recruiting to replace lost staff & sector expertise, & to cope with return to manual processes & increased demand. 7. Relationship Management: "...re establishing relationship management with our external providers, both in pilot mode, putting it back for NHS Trusts, and at the end of this month, commencing work with social care providers..." ➡️ Translation: Returning to named relationship managers for care providers. 8. Local authority assessment “We are into reflection mode on that work." ➡️ Translation: CQC will perhaps start listening to feedback from LGA, ADASS, providers.
