Addressing health disparities isn’t just about fairness—it’s a strategic imperative. By reducing barriers to care and empowering underserved communities, we create a stronger, more effective NHS that delivers better outcomes for everyone. In her Health Service Journal article, our Head of Partnerships and Involvement, Sarah Tilsed, highlights actionable steps NHS leaders can take to address these systemic issues: 1️⃣ Expand accessibility: bring screening, diagnostic, and treatment facilities closer to underserved communities to reduce travel barriers and improve early diagnosis. 2️⃣ Strengthen patient partnerships: listen to patients’ lived experiences, empower shared decision-making, and rebuild trust through inclusive, person-centered care. 3️⃣ Combat discrimination: implement comprehensive staff training to address racism, unconscious bias, and inequities in care delivery. The NHS has the opportunity to lead with bold action. Let’s work together to ensure equity is at the heart of healthcare.

Another NHS Providers conference done and dusted. It’s been an insightful two days filled with impactful keynotes and valuable discussions on some of the most pressing topics facing the NHS. From leadership and innovation to patient-centered care and health equity, each session offered new perspectives and ideas to drive positive change. The announcement of league tables has understandably raised concerns about the potential impact on staff morale, particularly for those working in services that may find themselves lower on the rankings. A key theme throughout the conference was the importance of empowering leaders to act autonomously and drive change without needing constant permission. Building a culture where staff can learn by doing, and where people feel safe to try - to learn from their mistakes - is essential. So while accountability is important, it will be crucial that it’s balanced carefully to ensure that our teams feel supported, not undermined, and that the innovation and improvement that is needed, is not stifled through fear of failure and of appearing at the bottom of the rankings. A powerful thread emerged over the 2 days: the significance of top-class leadership at all levels of the NHS. As the 10-Year NHS Plan and the new reforms set out today by Wes Streeting take shape, I hope they will create opportunities to strengthen leadership across our health service. My key takeaways: 🌟A focus on continuous improvement and innovation, and the need to empower our workforce through compassionate leadership. 🌟Recognising that not all change leads to improvement. It’s vital to make room for learning from mistakes without fear of punishment. 🌟The call for patient-centered services that are designed around complex, individual needs. “Power to the patient” means rethinking how we serve. 🌟An investment in leadership is essential. We need values-based, outcomes-focused leadership that truly tackles health inequalities. Harnessing data and disaggregating will help us better understand our populations. 🌟Moving towards an earned autonomy model by trusting leaders to drive change, guiding staff on what to achieve and why rather than dictating how to do it, and fostering an improvement culture. 🌟Leaders need to lead with authenticity and compassion. At the heart of it all is “mattering”, ensuring our staff know their work is meaningful and impactful. 🌟 Prioritising staff wellbeing means not only providing resources but also fostering a culture where our people feel genuinely cared for. 📢 Tackling the NHS Workforce Crisis: A Focus on Primary Care 🩺 The UK Government is refreshing its NHS workforce plan this summer, 🌟 Press Release published a few days ago https://lnkd.in/eiuuVREW 🔑 Key challenges addressed: 📉 A 16% lower ratio of fully qualified GPs compared to other high-income countries. 🚨 A 5% decrease in community nurses since 2009. 👶 A 20% drop in health visitors since 2019. 🧠 Mental health nurse numbers only recently recovering to 2010 levels. 💡 The goal: Shift care closer to home, reduce hospital dependence, and ensure timely, world-class healthcare. A key focus will be building the workforce in primary care, the backbone of our NHS. 🏥✨ (Kind of been saying that for a while!) But are we ready to address commissioning cycles that limit innovation and tackle the secondary care focus within ICS workforce teams? For real change, we need primary care at the forefront of workforce planning and investment. While it's great to see plans to increase GP and clinical staff numbers, how are we tackling retention? Without addressing burnout and providing better support, we risk losing the very professionals we’re trying to grow. And with a shortage of educators and trainers, how can we effectively train and supervise new staff entering the system? Matthew Custance The current system for deciding which/ when/ where to build new hospitals in the UK clearly isn’t working, and needs to change. PFI in my view didn’t work either, as it bundled building, FM and other services into complex contracts and the PFI consortia quickly became good at stealing sweeties from tge kiddies letting contracts. Has anyone done a comparison of the end to end process for planning and building a new public hospital in UK vs a range of other developed countries with either social health insurance or Bevan systems? My hypothesis is that in the UK there is a much greater degree of political and treasury involvement, leading to delays, decisions which are sub-optimal at a local level, and excess time and money spent planning projects.

Collaboration with who? Real collaboration with community services - from the report “The NHS budget is not being spent where it should be – too great a share is being spent in hospitals, too little in the community, and productivity is too low” Known throughout my 20 years + in community but nothing has ever changed What about learning from others, dare I say it some private providers are already doing this… Drive productivity in hospitals. Acute care providers will need to bring down waiting lists by radically improving their productivity. That means fixing flow through better operational management, capital investment in modern buildings and equipment, and re-engaging and empowering staff. Tilt towards technology. There must be a major tilt towards technology to unlock productivity. In particular, the hundreds of thousands of NHS staff working outside hospitals urgently need the benefits of digital systems. There is enormous potential in AI to transform care and for life sciences breakthroughs to create new treatments. Contribute to the nation’s prosperity. With the NHS budget at £165 billion this year, the health service’s productivity is vital for national prosperity. Moreover, the NHS must rebuild its capacity to get more people off waiting lists and back into work. At the same time, it should better support British biopharmaceutical companies. Reform to make the structure deliver. While a top-down reorganisation of NHS England and integrated care boards is neither necessary nor desirable, there is more work to be done to clarify roles and accountabilities, ensure the right balance of management resources in different parts of the structure, and strengthen key processes such as capital approvals. Change will only be successful if the NHS can recover its capacity to deliver plans and strategies as well as to make them. And what about Social Care, both reforms need to be done in tandem And let’s include the voice of the patients . Patient engagement. The patient voice is not loud enough The NHS should aspire to deliver high quality care for all, all of the time. That not only means care that is safe and effective but that treats people with dignity, compassion and respect, making their experiences as positive as they can be. Yet patient satisfaction with services has declined and the number of complaints has increased, while patients are less empowered to make choices about their care. A familiar theme in inquiries into care failings has been patients’ concerns not being heard or acted upon. The NHS is paying out record sums in compensation payments for care failures, which now amount to nearly £3 billion or 1.7 per cent of the entire NHS budget. I believe the time has come for less talk and more action - let’s see these recommendations being implemented

The NHS in Devon has launched local engagement around the 10 Year Health Plan, which will run in parallel with, and support, the national Change NHS campaign.   Between November 2024 and 28 February 2025 members of the public and patients in Devon, as well as NHS staff and healthcare experts, are being invited to share their experiences, views and ideas for improving the NHS. People are being encouraged to fill in the local survey even if they have already completed the national one.
