90% of #healthtech companies I talk to aren't at the level of sophistication they need to be to get into the NHS. They usually understand it at face value. But the NHS has many dimensions. Maybe not quantum dimensions. But often invisible currents that mean cause and effect can feel unpredictable. I don't find the NHS unpredictable, and I think it's entirely winnable... "Why have you chosen to pivot into healthcare?" One of the greatest privileges I enjoy in Adopt-A-Doc is the opportunity to speak to a wide range of healthtech #founders. To note, over 90% of these founders come from a technical or commercial background - rather than clinical. One of the first questions that I always ask is: 𝐖𝐡𝐲 𝐡𝐚𝐯𝐞 𝐲𝐨𝐮 𝐜𝐡𝐨𝐬𝐞𝐧 𝐭𝐨 𝐞𝐧𝐭𝐞𝐫 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞 𝐨𝐟 𝐚𝐥𝐥 𝐟𝐢𝐞𝐥𝐝𝐬? Now bear in mind, many of these founders have actually left jobs that were: - In stable industries, - Very well compensated, - In larger, multinational corporations, 👉 To foray in healthcare which has its own sets of challenges. _______________________________ I have heard many great stories. They range from: - A personal or family experience with illness, and the deep dissatisfaction that came from it. - A desire to make things better having seen first hand how prevalent ill health is. - A strong motivation to use their skill set to build something impactful. All you need is an explanation? 🗣 ❓   I enjoyed the latest joint RSM Digital Health Section and @Dev&Doc podcast featuring Dr Annabelle Painter, Dr Josh Au Yeung, Zeljko K. on #XAI.   𝐓𝐡𝐢𝐧𝐠𝐬 𝐦𝐞𝐧𝐭𝐢𝐨𝐧𝐞𝐝 𝐭𝐡𝐚𝐭 𝐈 𝐟𝐨𝐮𝐧𝐝 𝐢𝐧𝐭𝐞𝐫𝐞𝐬𝐭𝐢𝐧𝐠: 🧠 We need to trust a solution to accept it's decisions, and part of trust is understanding why - therein lies why we find explainability so important but actually it really boils down to our ability/willingness to trust?   🧠 We frequently use the terms ‘#explainability’ and ‘#interpretability’ with respect to AI as interchangeable, but they mean 𝘥𝘪𝘧𝘧𝘦𝘳𝘦𝘯𝘵 things (see: https://lnkd.in/eKuNMbXx.)   🧠 When we talk about explainable AI – do we mean explainable for each decision or how the system as a whole is working or both? And are they equally important? Also, could we even understand it or find it relevant (e.g. complex mathematical reasoning).   🧠 Whilst we expect AI to be explainable, is this a double standard when we don’t even know sometimes how humans make decisions? 🤷‍♀️ When referring for specialists/tests sometimes we have ☑ that says ‘clinical concern’ for when we can’t explain why a patent needs help – so if we can’t always explain our own intuition and make allowances for that, maybe we need to for AI? 🤔   💡 Fascinating point re: whether having an explanation makes doctors more likely to accept a false result. So could this actually worsen our over-reliance to AI? 😨  Something I saw raised in a Nature letter here published on explainable AI in dermatology. Lauren Oakden-Rayner gave a lecture several years back and discussed a 2015 study about how pigeons 🐦 could detect breast cancer on pathology images (https://lnkd.in/eD3beHN2 😂 ) and argued how they could be seen as a kind of ‘model’ outputting binary classification with high accuracy but we’d never dream of thinking of a 🐦 = 👩, yet we do this comparison with AI.   🤖 The topic of human-AI interaction and decision making cropped up a few times as an area of much needed research – although I’m certain there must be a lot out there given all the courses/degrees/jobs related to #HCI and #UX? No?   Anyway - lovely episode! Would enjoy hearing from anyone working on exciting projects that cross behavioural & social science / medtech. Always a fan of Freakonomics and how predictably unpredictable we can be with decision making…!

Can LLMs be truly explainable? What are the methods we can use to understand how a model works? Is explainability a essential requirement for Clinical AI? 🤔 🔹This episode was a merging of Dev and Doc and Dr Annabelle Painter from Royal Society of Medicine's digital health section podcast. (it was only a matter of time for these two great podcasts to join forces 😉) 🔹We sit down to deep dive into the fascinating topic of explainable AI, methods to interpret models, research on how we can understand deep learning models and LLMs. 🔹 As usual Dev Zeljko K. gives us an amazing technical overview of old and modern approaches including heat maps, pertubations, LIME techniques 🔹We highlight the importance of behavioural science of the clinician AI relationship, how that can lead to automation bias and "placebic information". 🔹It was a lot of fun and a pleasure to sit down and chat together. Can clinicians that are still working clinically contribute significantly to the healthtech industry? 𝐖𝐞 𝐛𝐞𝐥𝐢𝐞𝐯𝐞 𝐭𝐡𝐚𝐭 𝐭𝐡𝐞 𝐚𝐧𝐬𝐰𝐞𝐫 𝐢𝐬 𝐚 𝐫𝐞𝐬𝐨𝐮𝐧𝐝𝐢𝐧𝐠 𝐘𝐄𝐒. Having now spoken to over 110 healthtech start-up founders via Adopt-A-Doc, we know first-hand that clinical experience and expertise is in great demand. 🤔 The common sentiment is that you have to leave medicine to be able to play a significant part in HealthTech. And while that is true in some cases, I believe that we have not yet tapped into a vast reservoir of potential: 👩‍⚕️ 𝐂𝐥𝐢𝐧𝐢𝐜𝐢𝐚𝐧𝐬 𝐭𝐡𝐚𝐭 𝐚𝐫𝐞 𝐚𝐜𝐭𝐢𝐯𝐞𝐥𝐲 𝐬𝐞𝐫𝐯𝐢𝐧𝐠 𝐰𝐢𝐭𝐡𝐢𝐧 𝐭𝐡𝐞 𝐡𝐞𝐚𝐥𝐭𝐡 𝐬𝐲𝐬𝐭𝐞𝐦 𝐭𝐡𝐚𝐭 𝐡𝐚𝐯𝐞 𝐬𝐢𝐠𝐧𝐢𝐟𝐢𝐜𝐚𝐧𝐭 𝐜𝐥𝐢𝐧𝐢𝐜𝐚𝐥 𝐞𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞. 👨‍⚕️ __________________________________ 💡 Fun fact. Out of the 20+ paid piece of work we have delivered, every single start-up we worked with were looking for 𝘴𝘦𝘯𝘪𝘰𝘳 𝘤𝘭𝘪𝘯𝘪𝘤𝘪𝘢𝘯𝘴 who understand the pathways in which they are hoping to innovate in. 😄 And rightfully so, 1. Only a #clinician who is embedded within a system and undergoes the pain points daily will be able to articulate clearly what the problem is. 2. It is so much easier to get anything done within the #NHS with a clinical champion on the ground. 3. There is a huge element of credibility and legitimacy that comes with working with a highly experienced clinician. ☝ 𝐀𝐥𝐥 𝐭𝐡𝐞 𝐚𝐛𝐨𝐯𝐞 𝐚𝐫𝐞 𝐡𝐢𝐠𝐡𝐥𝐲 𝐝𝐞𝐬𝐢𝐫𝐚𝐛𝐥𝐞 𝐭𝐨 𝐟𝐨𝐮𝐧𝐝𝐞𝐫𝐬 𝐨𝐟 𝐚𝐧𝐲 𝐬𝐭𝐚𝐠𝐞. __________________________________ But there are definite challenges when it comes to tapping into this resource. 1. Clinicians are really busy and are naturally sceptical. 2. Both parties don't speak the same language. 3. There are tangible risks for both parties. AI Revolutionizing Healthcare: The Concept of Deep Medicine In the UK's National Health Service (NHS), challenges such as time constraints, limited resources, and overstretched staff often result in incomplete patient data and less accurate diagnoses. In his 2019 book, American cardiologist Eric Topol introduced the concept of "deep medicine," advocating for the use of artificial intelligence (AI) to transform healthcare. The deep medicine framework, as proposed by Topol, comprises three core pillars: deep phenotyping (comprehensive patient data), deep learning (AI analysis of complex data), and deep empathy (AI streamlining of administrative tasks and fostering better patient-staff relationships). 2025 will be a referendum year for Digital Health startups - as true “Substance” separates itself from the overfunded “Hype” of 2021/2022 👇 The last few years were both Good and Bad for Digital Health. The Good was that interest in Digital Health accelerated and health systems tried lots of new things. The Bad was that the bar for funding was often low… sometimes SO low that money was wasted - either on health systems paying for stuff that didn’t work or on startups “over-eating” investor funding. But by 2024, the healthcare market started to wake up to the idea that you just can’t pay for or fund cool ideas. You couldn’t just buy-in to Hype. Unfortunately that meant Digital Health startups that viewed pilots with brand name logos and millions in funding from famous investors as “success” started to pay the price. It also meant Digital Health deployments funded by grants, which didn’t solve real problems, started dying off too. Hype is all fun and games… until the reckoning comes. And now that the reckoning is coming, all that matters are Digital Health solutions with real Substance: → Deployments that actually go-live on time → Solutions that actually fit the clinical workflow → Innovations that actually have proven integration partnerships with EHRs → Solutions that deliver proven ROI via improved clinical outcomes and financial performance So yes 2025 will be the year Digital Health swallows some tough medicine. But we will be better off for it. Not only because the money isn’t there anymore to fund the Hype… … but because our patients deserve Substance. Caveat: I might be totally wrong since the AI hype emerged in 2024 - but I trust my CMIO and CIO friends to tame that beast 😁

𝗛𝗼𝘄 𝗦𝗵𝗼𝘂𝗹𝗱 𝗪𝗲 𝗥𝗲𝗴𝘂𝗹𝗮𝘁𝗲 𝗚𝗲𝗻𝗲𝗿𝗮𝘁𝗶𝘃𝗲 𝗔𝗜 𝗶𝗻 𝗛𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲? 🏥💡 As generative AI technologies like #ChatGPT and others rapidly advance, regulators face a unique challenge in adapting traditional frameworks for healthcare. Current FDA regulations, designed for specific drugs and devices, fall short when applied to AI models with vast and evolving capabilities. A novel approach is needed — one that treats these AI systems more like human clinicians, requiring rigorous training, supervision, and periodic reassessment. While challenges such as AI "hallucinations" and transparency in training data remain, a thoughtful regulatory framework can ensure these technologies safely enhance clinical practice. I was recently invited to deliver three keynotes during the 82nd International Pharmaceutical Federation (FIP) World Congress of Pharmacy and Pharmaceutical Sciences 2024 in Cape Town, South Africa from 1-4 September. During these sessions, I emphasized that we need a completely new approach and framework to regulate AI in healthcare — fully aligned with these insights. “How is AI being used to innovate in health?”, “What should startups do with AI to make themselves appealing to the NHS and corporate partners?” and “How can we align values?” - were just some of the questions posed at the AI in health event today hosted by Plug and Play UK and IBM. Oodles of enthusiasm was punctuated by a few nuggets of wisdom from panelists Malcolm Fogarty, Dr Arrash Yassaee, and David Browning. As someone who’s worked with AI/data/strategy for a long while, it was great to meet fellow founders and to learn about IBM’s startup support. Many thanks Dawn Herndon, Mark Osborn, and Agata Maria Bendik. As with all the AI events I’ve been to recently though (there have been many), I have a distinct sense of déjà vu: “AI powered” is a bit like the terms “tech enabled” or “digital” or “online” were in recent years gone by…. “AI powered” is a term that’s still used as a selling point or differentiator.
