Document Upload

  1. Make sure each section of the verification form was filled out properly, all forms, tax returns, statements were signed and dated.
  2. Specify the document type you are submitting. If your document type is not listed here, choose Other Document.
  3. It is important you choose the correct document type for each submission.
If the pages you are submitting belong to one document, combine those pages into one document first, and submit the final combined one document. The following are helpful tips for how to combine pages.

  2. If you don't have a dedicated scanner you can use your phone to scan documents. An example of a free utility that can be used for this is TinyScanner.
The accepted filetypes are png, pdf, jpg, jpeg, doc and docx. Please ensure documents are not password protected.
Use the "Drop File Here" or the "Add File" to upload the requested document.
Only attach and send one attachment at a time. If you have more documents to submit, on the next page, click your browser's back button to come back to this form to submit the next document. Remember to change the file document type each time.
