⭐️ 𝗧𝗵𝗲 𝗿𝗼𝗹𝗲 𝗼𝗳 𝗮𝗻 𝗜𝗖𝗨 𝗱𝗶𝗮𝗿𝘆 𝗶𝗻 𝗵𝗲𝗹𝗽𝗶𝗻𝗴 𝗿𝗲𝗰𝗼𝘃𝗲𝗿𝘆 𝗳𝗿𝗼𝗺 𝗱𝗲𝗹𝗶𝗿𝗶𝘂𝗺 David Richards @ICU_Diaries survived a severe case of ARDS in 2009-2010. As part of this he experienced severe delirium. In a really informative paper he describes how his recovery was in part attributable to a detailed diary kept during his 6 weeks in ICU. The diary was mainly kept by his partner. The diary has both handwritten notes & photographs. It includes: - Clinical updates on his condition - Conversations with family and medical staff - Richards' reactions and attempts at communication as he emerged from a 4-week coma, initially with delirium & fragmented memories Richards describes the diary as a lifeline that grounded him, helped him sift through distorted memories, & ultimately make sense of his awful ordeal. It allowed him to: - Trace a timeline of events, piecing together his fragmented memories of his time in ICU - Distinguish real memories from hallucinations - Grasp the severity of his illness through early entries that documented his critical condition & the real possibility of death The diary also provided a window into the emotional landscape of his illness, documenting: - His vivid nightmares and delusions which he later linked to specific medical procedures, such as the insertion of chest drains. - The profound psychological impact of ARDS, including fear, delirium, and paranoia. - His deep longing for his children and the central role of family connection in his recovery. 3 Takeaways from my recent experience at King's College Hospital London – UAE 1. A collective act of humanity. No matter where we are in the world, sharing knowledge and skills is deeply fulfilling. Helping a colleague, another hospital, or even a different country in their time of need is more than just a professional duty—it’s a humanitarian one. Contributing to the global collective good brings both contentment and purpose. 2. Aren’t patients our best teachers? Every case is a lesson, and my recent case of transjugular intrahepatuc portisystemic shunt -TIPSS procedure at King’s College London, Dubai reminded me how much we learn directly from the patient’s unique anatomy and disease process. Each encounter teaches us what to do, what not to do, and leaves us in awe of the human body’s intricacies. 3. Magic happens outside the comfort zone. Stepping into a new environment, especially for a procedure like TIPSS, comes with surprises. But isn’t that where growth truly happens? We only push our limits when we take that leap, allowing us to achieve more than we thought possible. All’s well that ends well! ✳️ A special note about King’s College London, Dubai- it’s not just a world class hospital that sets it apart but the willingness to collaborate is exemplary - The collaboration between experts and cutting-edge facilities creates an environment where both patient care and continuous learning thrive. The latest Getting It Right First Time (GIRFT) research, published in the British Journal of Surgery, highlights significant variation in surgery postponement rates across NHS England trusts in England, ranging from 1% to 32% (average 7%) based on data from 8,000 cases at 16 trusts. The study emphasizes the importance of earlier risk stratification and optimization pathways to reduce delays and enhance perioperative efficiency. As a trusted NHS partner, The Medical and Diagnostic Care Group works closely with multiple Trusts, tailoring solutions to address their unique challenges—whether tackling surgical and outpatient backlogs or improving theatre utilization to deliver better patient outcomes. Complex EMR of a large JNET 2A LST-M (TVA -LGD) in sigmoid colon performed during a #bowelcancerscreening list at Barking, Havering and Redbridge University Hospitals NHS Trust #DDC. Having a systematic approach is 🔑 1. Careful underwater lesion assessment in all body positions. Use virtual or traditional chromoendoscopy. 2. Use the correct scope and make sure you have everything you need. 2. Decide your strategy- change the patient's position, combine techniques (U-EMR and conventional EMR) 3. No panic when you get intra-procedural complications (RDI use essential) 4. Treat edges with STSC to avoid recurrence 5. Consider closing defect or hemostatic gel to avoid post-procedural bleeding

See link below 👇 https://lnkd.in/eyDmsGZd It was a great team effort and I am honoured to have had the opportunity to take part in this. 🔸User feedback and changes in opinion of healthcare staff over time working with #AI in a real world setting is an important factor to recognise 🔸 We found that staff change their attitudes to #AI inc. their perceived benefits and drawbacks after having experience of it 🔸 We saw that staff were more willing to have #AI used on themselves if they were a patient over time 🔸 Not everything was perfect. It is therefore important for anyone implementing #AI to keep communication channels open and re-evaluate how the tool is used in the care pathway to ensure ongoing clinical effectiveness. There was a lot of support and interest in this study when I presented it recently at EuSoMII - European Society of Medical Imaging Informatics in Pisa and a general sentiment amongst attendees that more attention should be given to human-AI interaction and human factors in the coming years when thinking about #AI influence in healthcare.

Very interesting session, exploring advancement of robotic surgery and the future of AI integration in surgical robots. The trajectory of surgical robotics to greater autonomy is inspirational, STAR ( soft tissue autonomous robots ) is already in clinical use outperforming human surgeons in precision during specific task. AI integration in robotic surgery is the way to democratize the surgical care. 🙏🙏🙏🙏🙏🙏🙏

🌟 Ending a Year Full of Fascinating Clinical Cases with a Remarkable One As this year draws to a close, which has been filled with numerous intriguing clinical cases, I want to highlight one particularly remarkable one that showcases the complexity and wonder of the human vascular system: - Right aortic arch - Aberrant left subclavian artery with Kommerell's diverticulum with associated tortuosity of the subclavian artery (<50% stenosis at the origin) - tracheal compression The combination of these anomalies led to a subtle yet significant tracheal imprint, which was studied non-invasively in detail using virtual bronchoscopy. As we approach the end of the year, it’s a great moment to reflect on the continued advancements in medical imaging and technology that help us provide better care for our patients. Our latest paper of our ongoing project has just been published at the Neurological Research, Taylor & Francis Group under the title; Stroke in Qatar: a decade of insights from a national registry Congratulations to the project team members; Yahia Z Imam Abdulqadir Nashwan Dr Naveed Akhtar Ibraheem Alkhawaldeh Ibrahem Albalkhi Said YaseenAhmad Hassanat @Dr. Ahmad Altarawneh, Dr. Ahmad Own. This publication marks a significant milestone in understanding stroke disease in Qatar and advances the prediction of its outcomes, moving towards personalized stroke care. Conducted at Hamad Medical Corporation - Stroke Service. This retrospective study analyzed data from 15,859 patients between January 2014 and July 2022, providing valuable insights into the characteristics and outcomes of stroke patients in Qatar. Key Findings: - 70.9% of patients were diagnosed with a stroke, with ischemic strokes being more prevalent. - Higher stroke-related mortality rates were observed in female patients compared to male patients. - Significant improvements in disability outcomes were noted, with 59.3% of patients having favorable outcomes at discharge, improving to 68.2% after 90 days. - Our study underscores the potential of machine learning based predictive algorithms in improving stroke management and outcomes.
