請教各位大神GOOGLE表單問題

各位大神好
請問GOOGL的表單是否可以一次取代裡面的文字
並且一併更改回應的試算表表頭呢?
因為每一個月都要填表，需要一個一個改很多種
如下圖(是例圖)

個人會考慮:

表單內不指定, 而是表單標題更新,
每個月到指定日期後, 從後台把前一個月的紀錄轉出處理.
