M&A and turnaround of healthcare facilities!   Took the Masterclass on 28 September.   India is the fastest growing major economy. It is the fifth largest economy and the third largest in terms of PPP. Healthcare industry is one of the biggest employers employing ~7.5 million people. Current size of the healthcare industry is $372 billion and is expected to increase to $638 billion by 2025, growing at a CAGR of 16-17%. India still spends less than 5% of its GDP on healthcare, as against the OECD countries 8-10% and the US a whopping 20%.   Most legacy providers are consolidating aggressively and almost 30000 crores has been invested over the last 4 years with some mega deals have also been signed, powered by PE firms. Part of this investment could be used to extend delivery to Tier2/3 towns wherein the care delivery remains abysmal.   Integration of the newly acquired healthcare facilities (brownfield projects) becomes paramount for the providers/ PE firms to cerate value for their stakeholders. It entails design and execution of a prudent comprehensive business strategy.   Greenfield healthcare projects present a unique challenge to operationalize and turn profitable since all expenses are front loaded. In the absence of comprehensive strategy, turnaround is well-nigh impossible.   I’ve brought out key highlights of the same during my masterclass. In recent years, the Indian government has introduced significant reforms to strengthen the healthcare sector. A key initiative, Ayushman Bharat, aims to enhance the health system comprehensively from primary to tertiary care, marking a shift from vertical health programs. Its first component focuses on building a next-generation primary healthcare network through Health and Wellness Centres (HWCs) to promote good health and enable early disease detection, particularly for non-communicable diseases (NCDs), which are rising in India. These HWCs prioritize disease prevention and extensive screening, offering sustainable solutions to reduce disease burden and minimize out-of-pocket healthcare expenses, which currently make up more than 60% of health costs. Additionally, the National Digital Health Mission (NDHM) seeks to digitize health records and create an efficient system for managing and exchanging health data. This initiative will help develop registries of healthcare providers, laboratories, and pharmacies while supporting clinical decision-making and enabling services like telemedicine. How does India deliver “Care” to rural communities through Digital Health? The mission of an ideal Digital Health Solution should be to deliver hospital-grade care, anytime and anywhere across India & the globe. The Solution should enable multiple health screens & deliver high-quality care regardless of whether the rural community has a local Doctor or a trained Nurse or whether they have power or WiFi, etc. While challenges like power & WiFi can be addressed through technology, how does one deliver health screens & diagnostics without the presence of a local Doctor or Nurse? Enter the ASHA Worker. THE most critical “Non-Technology” component of our Digital Health Solution. It is the Asha worker who ensures that all tasks related to creating a patient profile, capturing patient symptoms, and measuring patient vitals, are completed efficiently despite the unavailability of qualified medical professionals. The Asha workers also visit patient’s homes to provide door-to-door care – especially for older patients with mobility issues and for expectant mothers who may not be able to visit a clinic. And finally, there’s an emotional element to delivering good quality “patient care” that cannot be fulfilled by ANY technology, and which is warmly delivered by the Asha worker. Default Opt-In for Organ Donation will save countless lives! Great article by Srinath Sridharan in Mint yesterday recommending on Default Opt-In for Organ donation. NephroPlus fully endorses this opinion and the same has been communicated multiple times in various Healthcare forums. We, in fact, celebrate Transplants in our Dialysis Clinics. Transplant offers the best quality of life for people with Kidney Failure. Dialysis is only a second best option if Transplant is possible (considering various clinical factors) for anyone. Absolutely no doubt about this. Nordic countries have made organ donation as Default Opt-In which saw their number of registered Organ donors grow by orders of magnitude. People who do not wish to donate organs can Opt-Out for which they needed to fill a simple form with couple of questions. Brilliant move by Governments there since most people do not want to answer multiple questions and go by Default option. In India, a very effective option would be to link Default Opt-In in Aadhaar Cards! Other countries can use similar Government identification options - for example, US uses Drivers License Option (But it was not Default Opti-In when I signed up for it 15 years ago). 🏥 We need an Independent Indian Healthcare Administrative Services (IHAS) Cadre. The Indian healthcare sector is at a pivotal turning point, demanding swift and strategic reforms. The current landscape features IAS officials lacking specialized knowledge in healthcare administration occupying the highest position, which hinders effective communication and collaboration with medical professionals. The current Heads of Healthcare Institutions' experience in their respective specialties is unmatched, but their administrative and managerial expertise remains limited. An independent IHAS cadre, comprising medical, dental, and allied health sciences professionals, will ensure holistic management, fostering seamless collaboration and innovative solutions in healthcare administration. Without IHAS, communication gaps and administrative inefficiencies will continue to plague our healthcare system, risking patient welfare and operational efficiency. As India's healthcare expenditure approaches ₹90,171 crore, establishing IHAS is crucial. Entry-level requirements should include MBBS, BDS, or BAMS to leverage deep clinical and administrative expertise, revolutionizing the management of the healthcare sector in India.

Four times a week. That’s 208 cheat meals a year, minus the guilt, but plus the calories and chaos. What changed? The Internet, impulsive buying, and those food delivery platforms mushrooming on every corner. Unchecked kitchen hygiene, endless discounts, and clever marketing have turned “convenience” into a ticking time bomb for our health. Let’s face it—what my mom cooked in her tiny kitchen with love, care, and actual hygiene standards cannot be compared to what’s churned out in some of these places. But hey, it’s fast, it’s cheap, and it comes with “free delivery” (read—a health bill later). The result? Health checkups at 18? Hell, they were meant for 40. While we have made significant strides in renewable energy deployment and climate commitments, the pace of implementation is hindered by limited access to cutting-edge green technologies. This technology gap isn't just a matter of industrial development—it's a crucial factor in our nation's ability to meet its climate goals while sustaining economic growth. Why India's Success Matters Globally?? India's role in global climate action cannot be overstated. As the world's third-largest energy consumer and a rapidly growing economy, our success or failure in implementing climate solutions will have global ramifications. Consider these factors: 1. Our renewable energy target-500 GW by 2030 represents one of the world's most ambitious clean energy transitions 2. India's urban infrastructure development over the next decade will lock in emissions patterns for generations 3. This could include joint research programs, technology sharing agreements, and capacity-building initiatives. *Domestic Innovation Ecosystem- India must simultaneously strengthen its domestic innovation ecosystem. This means increased R&D funding, industry-academia partnerships, and policy support for clean technology startups. *Financial Mechanisms- Innovative financing solutions are crucial. Green bonds, climate funds, and public-private partnerships need to be scaled up to support technology adoption and deployment. India can lead by example in showing how developing nations achieve sustainable growth while fighting climate change.
