After Hamas' attack, I put down in words my feelings about Israeli policy. During those days, I think I was the only one who criticized Israeli policy. I did it openly. Some people called me crazy, but now they support my words. That's what I posted months ago: https://lnkd.in/dGCkUPKp Bellow, I repost my words again: "I carried out a technical analysis of local practices that encourage social and economic disparity between Israel and other groups. I don't want you to agree with my words, but imagine how restricting the population can affect people emotionally and socially. Imagine the situation below in your personal lives; definitely one day the explosion of hatred would occur. I am strongly against any type of attack on civilians, however, so it does not happen again in the future, we need to analyze the historical facts that raised rancor and anger among groups. By the way, I don't support Israel. I don't support Hamas. I will always support understanding and peace. One sentence: Peace cannot be built on severe and extreme exclusion. As the apartheid regime in South Africa, Israel justifies these measures under the pretext of 'security'. Contrary to such claims, they are in fact part of an overall regime aimed at preserving demographic superiority of one racial group over the other in certain areas. Israeli practices: - Violations of international standards for due process (such as illegal detention); - Discriminatory privileges based on ascribed ethnicity (legally, as Jewish or non-Jewish); - Draconian enforced ethnic segregation in all parts of life, including by confining groups to ethnic "reserves and ghettoes"; - Comprehensive restrictions on individual freedoms, such as movement and expression; - A dual legal system based on ethno-national identity (Jewish or Palestinian); - Denationalization (denial of citizenship); - Special system of laws designed selectively to punish any Palestinian resistance to the system. AI for backend, administrative workflows is low-risk and low hanging fruit. AI for direct patient care is higher-risk and needs more thoughtfulness when trying. The closer we get to the patient, the higher the risk, and the more thoughtful (and thus slower) we must be. 2. For clinical care, clinicians are most open to AI as a co-pilot. Similar to having a medical trainee with you at all times, and the physician still has to sign off on things. 3. While various AI co-pilots may enhance patient care and clinician well-being, the data shows many don’t necessarily save time. 4. Autonomous AI agents that own end-to-end clinical care are much further away. Technologists underestimate the clinical complexity of many clinical scenarios, the risk/benefit trade offs are not yet well-studied and the liability implications are unclear. 5. AI medical scribes and drafting patient portal messages are by far the two most common Generative AI use cases being tested and implemented right now. 6. Adoption of AI Scribes by clinicians has been mixed - it’s game changing for some, but it’s not been for everyone. The value isn’t increased patient throughput, but rather improved clinician well-being. 7. Generative AI for drafting patient portal messages also has had mixed results on adoption - also not for everyone. 8. The current AI roadmap is primarily driven by the EHR as lots of the opportunities for improvement are reducing the administrative burdens… created by the EHR in the first place. 9. AI prediction models are only useful if the insights are actionable, fit the clinical workflow, and align with drivers for adoption. Which means many AI models make great research publications but never actually get implemented at scale. 10. Got into a rabbit hole learning more about the Israeli health system. Some (fun?) facts: - Israeli’s population is ~9.5 million - There is universal coverage, but each person can choose between 4 competing non-profit plans  - 85% of people opt to purchase additional private insurance for dental, increased access to providers, or other amenities Primary & Speciality Care - The 4 plans have different approaches. One (Clalit) owns and operates it’s own PCP clinics, while the others use a mix of interdisciplinary clinics and independent PCPs  - No quality measurement financial incentives are used - Clinic based physicians under (Clalit) are paid a salary negotiated via collective bargaining agreement with the Israeli Medical Association - All plans use capitation rates. There are “active” vs “passive” capitation approaches. Active = per-member-per-quarter payment regardless if the patient visits. “Passive” is paid regardless of visit. Hospital Stuff - Hospital emergency care is reimbursed on a fee-for-service basis. - Inpatient medical care is reimbursed on a per-diem basis. - Inpatient procedures are paid either per diem or through activity-based, procedure-related group (PRG) arrangements - Outpatient care is reimbursed on either a fee-for-service or a procedure-related group basis. - Maximum rates are set by the gov, but plans negotiate discounts  - Hospitals have set revenue caps and revenue parameters set by the government to limit year to year fluctuations Was surprised to see that quality measurements were not generally used but then thought about it more and realized it makes sense (Goodhart’s law). One bigger million (billion?, trillion?!!) dollar question is whether aspects of this approach can scale up over 100x to help the US system work better. Maybe? 𝐑𝐚𝐦𝐳-𝐞-𝐆𝐥𝐮: 𝐀𝐧 𝐈𝐧𝐭𝐞𝐥𝐥𝐢𝐠𝐞𝐧𝐭 𝐃𝐞𝐯𝐢𝐜𝐞 𝐟𝐨𝐫 𝐃𝐢𝐚𝐛𝐞𝐭𝐢𝐜 𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭 "Ramz" ( رَمز ) is an Arabic word meaning "something that contains secret information," and "Glu" stands for "Blood Glucose Level." Ramz-e-Glu is an intelligent medical device that 𝙣𝙤𝙣-𝙞𝙣𝙫𝙖𝙨𝙞𝙫𝙚𝙡𝙮 𝙢𝙚𝙖𝙨𝙪𝙧𝙚𝙨 𝙗𝙡𝙤𝙤𝙙 𝙜𝙡𝙪𝙘𝙤𝙨𝙚 𝙡𝙚𝙫𝙚𝙡𝙨, 𝙗𝙡𝙤𝙤𝙙 𝙥𝙧𝙚𝙨𝙨𝙪𝙧𝙚, 𝙖𝙣𝙙 𝙝𝙚𝙖𝙧𝙩 𝙧𝙖𝙩𝙚 by acquiring Biosignals from the index finger. As part of our 𝐅𝐢𝐧𝐚𝐥-𝐘𝐞𝐚𝐫 𝐩𝐫𝐨𝐣𝐞𝐜𝐭 (BE Biomedical Engineering), we designed and developed Ramz-e-Glu according to international standards. We collected a dataset from 101 subjects, performed digital signal processing, and extracted features from the acquired biosignals. After completing feature engineering, we trained several Machine Learning based regressors/classifiers and deployed robust AI models, along with an interactive app for real-time measurements. After testing, results were compared with existing studies, and it was concluded that 𝑹𝒂𝒎𝒛-𝒆-𝑮𝒍𝒖 𝒔𝒐𝒍𝒗𝒆𝒔 𝒕𝒉𝒆 𝒑𝒓𝒐𝒃𝒍𝒆𝒎 𝒐𝒇 𝒍𝒐𝒘 𝒂𝒄𝒄𝒖𝒓𝒂𝒄𝒚 𝒊𝒏 𝒎𝒆𝒂𝒔𝒖𝒓𝒊𝒏𝒈 𝒃𝒍𝒐𝒐𝒅 𝒈𝒍𝒖𝒄𝒐𝒔𝒆 𝒍𝒆𝒗𝒆𝒍𝒔. This device can potentially reduce the pain and cost associated with invasive blood glucose monitoring, thus contributing to the achievement of the United Nations' Sustainable Development Goal of Good Health and Well-being. 🇮🇱🇱🇧 IDF spokesman: The IDF completed a wide series of attacks on military headquarters of the terrorist organization Hezbollah. A short time ago, the IDF launched an extensive attack on military targets of the Hezbollah terrorist organization where the organization's terrorists were. As part of the attack, dozens of warplanes, under the direction of the Intelligence Division, the Northern Command and the Air Force, attacked about 120 military targets of the terrorist organization Hezbollah in areas in southern and deep Lebanon. Among the properties that were attacked, significant headquarters of various units of the organization, military buildings and many infrastructures from which Hezbollah terrorists planned and carried out terrorist plots against the State of Israel. The attack constitutes another significant damage to the organization's competence, its command and control capabilities and its conduct in emergencies.

People die every day - unnecessarily - because we, humans, don't trust AI. This is another topic that came up in my "AI & Big Data Economics" Course @ Tel Aviv University. It seems that we are holding AI to superhuman standards in almost every field and implementing AI too slowly. One clear example is autonomous cars which are already significantly safer than human drivers (and getting better every day). AI is far from perfect, but already causing fewer accidents than human drivers. Yet, we are years away from handing over the keys and saving hundreds of thousands of lives a year globally. 🇮🇱Prime Minister Benjamin Netanyahu: What will change tonight, citizens of Israel? *That on this night, 133 of our dear brothers and sisters are not brought to the Seder table, and are still imprisoned in the inferno of Hamas.* We have already freed 124 of our abductees and we are committed to returning them all home - the living and the spaces alike. And what won't change tonight? that in every generation our brides rise above us, and the Holy One, blessed be He, saves us from them. *Unfortunately, until now, all proposals for the release of our abductees have been outright rejected by Hamas.* That is why the American Secretary of State rightly said: "Hamas has refused all proposals submitted to it." According to him, the only thing preventing a deal to release the abductees is Hamas. *Instead of retreating from its extreme positions, Hamas builds on division within us, it derives encouragement from the pressures directed at the Israeli government. As a result, he only tightens his conditions for the release of our abductees. He weighs down his heart, and refuses to send our people.* *Therefore, we will land additional and painful blows on him - and it will happen soon.* In the coming days we will increase the military and political pressure on Hamas because this is the only way to free our hostages and achieve our victory. As our soldiers are united to protect us on the battlefield, we are united to protect them on the political field.

Next Tuesday at the annual conference of the Israeli Society for Patient Rights, I'll be moderating a panel that I hope and expect will be interesting - on AI and its value for patients. Two themes will guide the panel: 1. What patients can gain from this new technology and what should they avoid - both now and in the future - #patientsuseAI 2.
