Make the timeout dependency conditional. [skip appveyor]

Tolerate a missing/broken "timeout" binary by default.  Print diagnostic
messages before starting the tests to make it clear which timeout binary
and value are going to be used and why.  Print errors to stderr
consistently and explain usage in more detail.
