It is only the first edition, but HLTH Europe has already established itself as the leading #HealthTech conference in Europe. Besides my 4 highlights, I also had the chance to moderate a panel on the topic "Last mile: Localising access for underserved areas" with 3 fantastic panelists: · Prathibha (President, Mayo Clinic Health System) · Minal (National Director of Primary Care and Community Transformation, NHS England) · Harmony (SVP, Chief Medical Officer & Head of Global Medical at ViiV Healthcare) PS: Amira (SVP Innovation at Siemens Healthineers): we missed you! Here are the key insights of the panel: Bringing healthcare #access to all, especially to underserved areas, has been a challenge since the inception of healthcare. With all the technology of our modern world, is #Healthcare access on the verge of being solved? The short answer is no. But there is hope. In the UK: ➝ There is a 15-year life expectancy gap between deprived areas, and wealthier neighbourhoods nearby. ➝ Funding struggles to keep up: In the next 15 years, a 37% increase in "ill-health costs" is expected, while the working population will increase by 4% over the same period. ➝ Increasing productivity is essential to solve the problem: A 1% decrease in productivity costs 1.5 billion pounds. In the US: ➝ The zip code is (still) a better determinant of health than the genetic code. ➝ Enabling access to underserved areas can also be seen as an opportunity rather than a problem. Tech adoption will be faster due to higher unmet needs, accelerating innovation that can later benefit well-served areas. Debunking one myth: ➝ For HiV, there is now a better access and coverage in Africa than in certain parts of developed countries (US and Europe). Pockets of innovation: ➝ To unlock HCP capacity, the NHS has a national program to move towards a modern General Practice, moving away from "first come first serve" to a new model based on population health and segmentation: the right care for the right person in the right way. First results show that 30% of GP appointments could be done via "messaging only" (instead of F2F visits), bringing massive efficiency gains. ➝ Mayo Clinic already has 75 specialties that are technology enabled, bringing the “Mayo medical care" in the rural areas via virtual and hybrid models, and also embedding the 50+ AI algorithms already active at Mayo. ➝ But increasing access is not only about Tech. Sometimes, it is about boldness: Viiv Healthcare, a pharma company specialised in HiV, gave the patent of its drugs away in 90+ underserved markets, enabling cheap and at-scale production. What does the future hold? Will someone living in an underserved area be able to visit the "Mayo Clinic in the metaverse with his / her digital twin?" SwissTech rocks: Watchibia , an innovative start-up led by Olga Nijpels-Tishurova imagines a smart strap that perfectly fits classical watch. TrueYouOmics vy measuring thousands of proteins and billions of potential mutations, their goal is to predict disease risks with unprecedented accuracy and correlate them with the patient’s current state. Visienco by using organoids—mini replicas of organs—to study diseases and test the drugs, aims to revolutionize personalized medicine. Their technology, developed in collaboration with CSEM experts, promises to significantly reduce the need for animal testing and speed up the development of innovative treatments. Proton therapy is considered the future of cancer care. Its benefits are undeniable: it not only reduces treatment costs but also significantly lowers side effects compared to conventional treatments. Protonica, a start-up idea developed by Veronica Leccese at CSEM and her colleagues Benoit Truc and Michele Caldara from EPFL, is a tool that aims to make proton therapy even more effective and accessible for everyone. This year, CSEM is particularly proud to note that one of the selected projects is led by engineers already within the organization. "#AI in #healthcare - opportunities, challenges and solutions" was the title of the session that Health Hub Vienna, LISA - Life Science Austria, INiTS | Vienna's High-Tech Incubator, EIT Health Austria held. It was, rightly, super overbooked. Georg Langs, with his contextflow and his Medizinische Universität Wien hats on, inspired us all with the way AI is affecting and will affect healthcare, from predicting disease course, responses to treatment, optimizing patient management, enabling secondary but also primary prevention (I will come back to that point), as it is already happening, but also to develop new therapies by identifying new early endpoints, incorporating patient satisfaction and QALYs, discovering new therapeutic targets, identifying new phenotypes, among others. In the last few years, there has been a shift from AI to support complex cases to AI to support healthcare efficiencies (starting to be of interest for payers for this reason at least), and narrow or close the healthcare workforce gap. Andreas Strouhal represented those many doctors that want to do their best for their patients, want to be able to recall and connect the dots from what they have read or heard on papers and conferences, and make it available at the point of care, right there, for the patient. AI not only to make the system more efficient and be able to see more patients per minute, per hour, but to be able to connect truly with them, to look at them in the eye, to put a hand on their shoulder. AI to augment and not to replace doctors. Helene Prenner, from ELGA GmbH but also in her own name, started with one of these statements that mark pitches "Data has no boundaries" and talked us through the challenges Europe is going through to make the most out of the fragmented data and use it for good, for individual patients, but also for research and to advance healthcare and society. Endri Deliu, bringing the US perspective, with a hopeful message Europe, said loud and clear to move faster, to try bold solutions, think much bigger, embrace change and failure and collaborate. He said we need fuel: talent, which Europe and Austria has, and money, and not "old-money" -as he called banks and insurance companies-, new money that invests and bets with Venture arms to speed up things and build the muscle. Regulation, -of course on the table-, may bring inequalities in the adoption of AI, bringing giants ahead of SMEs as they can afford employing people to make sure they comply with every line. To the question of using AI to shift from a sick-focused system to a health system, Helena replied with a call to collaboration, accounting for social-determinants of health. Irene Fialka, marvelously moderated the session, gave me the opportunity to ask a final question: how can we shape the present of the workforce challenge and attract the right AI and EI equip talent to the healthcare workforce for a sustainable future. VdTech rocks : Lausanne-based Precisia Care SA Care, use real-time clinical data directly collected from hospital operations to enable actionable AI-driven predictions and recommendations, has secured CHF 1 million in funding. In addition to this funding achievement, Precisia Care, incubated at La Forge part of the EPFL Innovation Park, was recognized as one of eight winners of the Future of Health Grant (FoHG), an initiative supported by EPFL Innovation Park and CSS, during its fifth call for projects. Tempus AI is interesting to monitor as it serves as a barometer for the HealthTech industry. ✅ A successful IPO would mark a significant milestone for the HealthTech sector, potentially the first of many successful IPOs to come. ❌ If the IPO fails, it indicates that the #HealthTech sector has (still!) not really emerged yet.

In November we launched our preventive care startup, Autonomic Health. The same week, Forward Health, a high-profile preventive care startup that had raised ~$700M from Founders Fund, Khosla Ventures, Salesforce’s Marc Benioff, among others, abruptly shut down after an 8-year run. Over the holidays, many people asked my take on why Forward failed and what we can learn from their mistakes. We had gone through the idea maze and studied the failures of previous preventive care startups before launching Autonomic Health. It’s the tale of P4 medicine (predictive, preventive, personalized, participatory) becoming O4 medicine (overtesting, overdiagnosis, overtreatment, overcharging). Coined in 2003 by Dr. Leroy Hood, the father of the biotech revolution, P4 medicine is the conceptualization of a new health care model based on multidimensional data and machine-learning algorithms in order to develop public health intervention and monitor health status of the population with focus on wellbeing and healthy aging. However, detractors talk about O4 medicine as a counterpoint to P4 medicine to highlight the drawbacks, including possible harms and cost. Dr. Hood’s own preventive care P4 startup, Arivale, shut down in 2019 after a four-year run and over $50M in funding.  Autonomic Health fixes all the problems that ailed these failed startups focused on preventing diseases. Most had unsustainable business models with fancy clinics and scaling flaws. We fortunately also benefit from favorable market inflection points (AI and wearables becoming commoditized/mainstream) to offer a frictionless and cost-effective approach to P4 medicine: * Back to the basics, first-principal approach: We monitor the autonomic nervous system, the body’s control center, on a 24/7 basis through any consumer wearable. Our frontier AI model performs 24/7 testing in a natural setting as you go on with your daily life, analyzing millions of contextual autonomic signals to assess your ongoing vitality and catch the earliest signal of disease. We will convert your wearable into a powerful tool to predict and prevent disease, uniquely for each person, and offer hyper-personalized care in real-time. * Low cost: No physical clinics. No expensive tests. All you need is a wearable device (prices are now <$25). * Scale: Anyone with a wearable device will be able to download our app to get started. We will instantly reach 1.2Bn people globally who already own a wearable device.  The moment for P4 medicine, 20+ years after it was proposed, may have finally arrived. Dr Hood himself predicted that "by the mid-2020s each individual will be surrounded by a virtual cloud of billions of data points and we'll have the computational tools to analyze this data and produce simple approaches to optimize wellness and minimize disease for each individual." Private capital has huge potential to help develop a local ecosystem. It is the fuel for the local engine (startups and founders) to work. However, in biotech, local ecosystem dont thrive isolated but connected to the larger ones. Florian Schuster and Christopher Trummer seems to be doing this at Venga Ventures. From the little I have seen here, the idea is to connect Austria to the larger biotech league. If successful, the value created by these companies is spreaded between all engaged countries. If I understood this correctly, I share the excitement of this strategy and believe there is great technology to be developed outside of the larger biotech places of the world (think Boston, San Francisco, New York, San Diego and London), but that do not have a fighting chance if not connected to them somehow. Some local VCs in Brazil and Latan are doing this too like Vesper Biotechnologies and Zentynel Frontier Investments but there is room for more. atai (Nasdaq: ATAI) went public almost exactly 3 years ago. I am fully aware that ATAI Life Sciences AG (as well as most other psychedelic stocks) are trading roughly at all-time lows. While I know this is painful for all shareholders, including myself, the share price doesn’t reflect what I believe the fundamental prospects are:   ·      While I understand that biotech stocks are especially sensitive to interest rates – and that’s why, IMO, many other high quality biotech stocks are suffering the same share price dynamics as atai – atai has 8 (!) clinical stage (!) drugs in development to help tackle some of the most prevalent mental health issues of our time.   ·      Biotech always has the risk that trials don’t work out due to safety and/or efficacy. However, atai’s pipeline has purposefully targeted those (psychedelic) compounds with prior human evidence, which provides valuable insight into their safety and effectiveness and de-risks its pipeline.   ·      atai is the only company featuring a pipeline that, IMO, contains all major psychedelics that both make sense for development as a drug and which are supported by a strong patent estate - namely, psilocybin / psilocin, DMT, 5-MeO-DMT and ibogaine.   ·      Plus 2 psychedelic-adjacent compounds: R-Ketamine and R-MDMA.   ·      Plus 2 non-psychedelic mental health drugs.   ·      I know that many expect that the roll out of psychedelic therapy will be slow, as we indeed need to educate doctors and patients about a new treatment paradigm. However, I have exactly the opposite opinion: psychedelics have already arrived in society (from “How to change your mind” on Netflix to every dinner conversation I have), and many, many people touched by psychedelics go on to become “advocates”. The demand is there and is eagerly awaiting atai’s success. In my opinion, those who believe psychedelics will have a slow roll out will be very positively surprised.   ·      Yes, Spravato’s roll out was initially slow, but IMO it was because they didn’t initially know how to handle the roll out for a psychedelic drug. It was also a different time. And to give J&J credit, they paved the way for the 2-hour treatment window most of atai’s psychedelic compounds are designed to fall into. And if you look at Spravato’s numbers today you see they are now growing substantially and anticipated to shortly hit “blockbuster” status.   Please check out my blog post from earlier this year for more information.   I know it’s not easy for shareholders to navigate this ongoing biotech winter. This is really interesting. We won the Health-X dataLOFT prize in Berlin. Really excited to start collaboration. Especially interesting is the EHDS angle. EHDS legislation was accepted in the European Parliament a couple of weeks ago. Centric in the legislation is how to facilitate secondary use of health data while safeguarding people's privacy. Health-X dataLOFT is tasked with developing a platform suitable for that in Germany and Europe. We are very excited to be a part of that journey! Why Patient Visits and Enrollment Estimates Are Completely Made Up (German Edition) I don’t know where to start. The conclusion might be a good introduction, though. Oncologists can’t tell you how many HER2-positive breast cancer patients visited their site last month. And if you want to know how many of them were in their 3rd line of therapy with prior PD-L1 inhibitors, they just can’t. This applies to any indication, criteria, biomarker, etc. Now you (sponsor/CRO) are asking sites to estimate the number of patients they are treating and how many of them will be eligible to be recruited. And the investigator will give you a number. Only, it’s a completely made-up number. Not because they are liars or because they want to fake the data to access the clinical research opportunity, but because you are asking them to provide a number they cannot provide. The EHR monopolies in Germany are messed up. By now, Patrick and I have had enough exposure to the UX and UI of patient records management at oncology clinics that after each demo from an investigator, we go silent, unable to exchange words. Let’s take Medistar from CompuGroup Medical Deutschland AG as an example. A company that scored $1.2 billion in revenue in 2023 with a product used by most clinical research sites. To answer the question of how many stage II breast cancer patients a site have seen in May 2024, the investigator or nurse will have to open each patient record on Medistar, scroll around, open the physician letter (a Word document), and read through it for the information. And if you want to know how many patients are, let’s say, “triple-negative” vs. HER2-positive, or if you are ambitious and looking for a specific line of therapy, you just keep reading. And if you average over 500 patients a month, you got few days of unpaid reading to do as an investigator. Sure, there is some “outsmarting the system” sites can do to have an estimate, like checking the prescription code of specific drugs. Still, that number narrows the field, mix the indications, might get you close but doesn’t give you the truth. So what can you do as an investigator? When you know you see the patient population required. No clue how many, because you work at least 12 hours a day, running from one room to the next, from one patient to the next. You would like to be part of the research. You like the IP. You like the sponsor. You write down a number. Something you think “it’s about right.” That’s all you can afford on a busy day, before the next patient enter the room, or you are asked to visit a room. - Site visit reflections #FoundersAtWork #ClinicalTrials #Oncology
